摘 要:目的了解联合检测乙型肝炎病毒(HBV)DNA阳性的乙型肝炎携带(ASC)、慢性肝炎(CHB)、肝硬化(LC)、肝癌(HCC)患者的HBV DNA病毒载量、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、甲胎蛋白(AFP)的意义,为临床诊断和治疗乙型肝炎提供参考依据。方法收集根据临床诊断为ASC、CHB、LC、HCC的HBV DNA阳性的乙型肝炎患者标本804例。采用实时荧光定量聚合酶链反应(PCR)法检测HBV DNA,采用速率法检测ALT、AST,采用化学发光法检测AFP,并对其结果进行比较分析。结果 HBV DNA定量结果及ALT、AST、AFP的异常率在男性和女性的各疾病组中的差异均具有统计学意义(P<0.05),HBV DNA定量结果在男性中以CHB组最高;女性中以ASC组最高,男性及女性均以HCC组最低;ALT异常率在男性及女性中CHB组均最高;AST异常率在男性以HCC组最高,在女性以LC组最高;AFP异常率在男性及女性中均以HCC组最高,ALT、AST、AFP在男性及女性中均以ASC组最低。结论 HBV DNA病毒载量与ALT、AST、AFP的联合检测对乙型肝炎患者临床诊断、治疗预后等有重要意义,尤其是ASC者中男性及女性的ALT、AST、AFP的异常率均不高,但HBV DNA病毒拷贝数均不低,建议密切监测这类患者病情变化。Objective To make comprehensive analysis on the results of alanine aminotransfeIase (ALT), asparate aminotransferase (AST) and alpha fetal protein (AFP) from the patients with hepatitis B virus (HBV) DNA of hepatitis B virus carrier (ASC), chronic hepatitis B (CHB), liver cirrhosis (LC) and hepatocellular carcinoma (HCC), so as to provide a evidence for clinical diagnosis and treatment. Methods Collected 804 of patients' specimens with HBV DNA copy, diagnosised as ASC, CHB, LC and HCC (4 groups). Detect all the HBV DNA with real time PCR, the content of ALT and AST with method of velocity, and the content of AFP with chemiluminescence. Results The quantitative results of HBV DNA and the abnormal rates of ALT, AST and AFP among male and female in 4 groups were statistically significant (P〈0.05). Male in group CHB had the hightest results in quantification of HBV DNA, while female in group ASC, and both male and female had the lowest quantitative results in group HCC. The abnormal rate of ALT was the hightest in group CHB. For male, AST abnormal rate was the hightest in group HCC, while in group LC for female. The group HCC had the hightest abnormal rates of AFP in both gender. The abnormal rates of ALT, AST and AFP were all the lowest in group ASC. Conclusion The content of HBV DNA and the joint detections of ALT, AST and AFP have significantly difference in clinical diagnosis, treatment and prognosis,especially the abnormal rates of ALT, AST and AFP of both gender hepatitis B carriers were not high, but the HBV DNA viral copies were not low. We propose that these patients should be closely monitored.
