穴位埋线联合静脉镇痛治疗混合痔术后疼痛的临床观察

摘　　要：目的观察对混合痔手术患者实施长强、承山穴位埋线联合静脉镇痛的临床疗效,探索新的混合痔术后镇痛方法。方法将120例混合痔患者,随机分为治疗组(长强、承山穴位埋线联合静脉镇痛组)和对照组(长强、承山穴位埋线组)。对照组仅予穴位埋线,治疗组在对照组的基础上,于手术结束后进行静脉镇痛。对术后疼痛始发时间、术后8 h、24 h、48 h、首次排便、3 d、7 d的疼痛强度、以及恶心呕吐、嗜睡、尿潴留等不良反应指标进行比较。结果术后疼痛始发时间、术后8 h、24 h、48 h、首次排便、3 d、7 d的疼痛强度,治疗组明显优于对照组(P<0.01)。但治疗组的不良反应发生率较高(P<0.05)。结论长强、承山穴位埋线联合静脉镇痛能有效改善混合痔术后疼痛,且镇痛时间更长,值得推广应用。Objective To observe the clinical efficacy of catgut implantation at Changqiang（GV1）and Chengshan（BL57）combined with intravenous analgesia for the patients with mixed hemorrhoid operation and explore a new analgesic approach to the postoperative pain of mixed hemorrhoid.Methods One hundred and twenty patients of mixed hemorrhoid were randomized into a treatment group[catgut implantation at Changqiang（GV1）and Chengshan（BL57）combined with intravenous analgesia]and a control group[catgut implantation at Changqiang（GV1）and Chengshan（BL57）].In the control group,only the catgut implantation was applied.In the treatment group,on the basic treatment as the control group,the intravenous analgesia was given after operation.The starting time of postoperative pain,pain severity in 8 h,24 h,48 h,the first defecation,3 d and 7 d after operation,and the adverse reactions such as nausea,vomiting,somnolence and urine retention were compared.Results Concerning to the starting time of postoperative pain and pain severity in 8 h,24 h,48 h,the first defecation,3 d and 7d after operation,the results in the treatment group were apparently superior to those in the control group（P〈0.01）.But,the incidence of the adverse reactions in the treatment group was relatively higher（P〈0.05）.Conclusion The catgut implantation at Changqiang（GV1）and Chengshan（BL57）combined with intravenous analgesia relieves effectively postoperative pain of mixed hemorrhoid and achieves much longer analgesic duration.Hence,this therapy deserves to be promoted in application.
