肩关节软组织炎症与正常肩部周围软组织的声像图比较

摘　　要：目的比较肩关节软组织炎症患者和正常肩部周围软组织的声像图表现,探讨超声在诊断肩关节软组织炎症性病变中的价值。方法对40例肩关节软组织炎症患者(病例组)肩袖、肌筋膜、肩滑膜囊及关节囊等部位进行超声检查,另选80正常成人(对照组)检测其相同部位,结果进行对比分析。结果病例组肩部的各型异常图像以病变区受累部位测值的增大、增厚、积液区范围广泛等为主要表现,同时伴随肌腱、腱鞘回声增强,水肿、钙化,肌筋膜毛糙、增生、回声紊乱等多种异常声像。与正常对照组比较,病例组受累肌腱明显增厚,差异有统计学意义(P﹤0.05)。肌筋膜炎、肩关节周围滑囊炎及粘连性关节囊炎病变区肌筋膜厚度、滑囊积液深度及肩关节囊下壁厚度均明显大于对照组,差异均有统计学意义(P﹤0.05)。结论超声检查对肩关节软组织炎症性病变可以清晰显示,其超声表现具有一定特征性。Objective To comprehend the ultrasonic appearance of shouder joint soft tissue inflammation and healthy shoulder peripheral soft tissue, and to explore the value of high frequency ultrasound in diagnosing shoulder joint soft tissue inflammation.Methods High-frequency ultrasound was performed in 80 healthy adult shoulders and 40 shoulders with soft tissue inflammation.Sonographic features of rotator cuff, muscular fasciae, shoulder bursa mucosa, joint capsule and other structures were evaluated.The differences between the two groups were compared.Results In case group,the main performance of various abnormal image in lesion area of shoulder joint consisted of increaseing,thickening and wide effusion area,with tendon and tendon sheath echo-enhanced,edema,calcification and muscular fasciae rough,hyperplasia,echo disorder and so on.Compared with control group, affected tendon thickened obviously in case group （P 〈 0.05 ）.In lesion area of myofascitis, bursitis around shoulder and adhesive synovitis, myofaseial thickness, bursal effusion depth and shoulder joint capsule wall thickness were significantly higher than the control group （P 〈 0.05）.Conclusion Ultrasonography can display clearly the pathological changes of shoulder joint soft tissue inflammation, and their ultrasonic appearance have certain characteristic.
