儿童股骨近端解剖钢板治疗儿童股骨粗隆下骨折的初步临床研究

摘　　要：目的:探讨新型儿童股骨近端解剖钢板治疗儿童股骨粗隆下骨折的方法和疗效。方法:采用新型儿童股骨近端解剖钢板治疗儿童股骨粗隆下不稳定骨折25例,记录手术时间、术中出血量,术后定期复查X线片,评价骨折愈合情况及髋关节功能。结果:本组25例平均手术时间55±10min;平均出血量为50±10mL;术后随访9～36个月,平均25个月,全部骨性愈合。未出现髋内翻、切口感染、内固定衰败或断裂等并发症。术后早期影像学按Beaty标准满意24例,满意率96%,晚期按Theologis标准满意率为100%,随访终末髋关节功能按Sanders标准优24例,良1例,优良率96%.结论:新型儿童股骨近端解剖钢板治疗儿童股骨粗隆下骨折具有创伤少、固定稳定、并发症少、符合儿童股骨近端解剖学特点等优点,值得临床推广应用。Objective：To explore the clinical effects of the anatomical proximal femur bone plate in the treatment of subtrochanteric fracture in skeletally immature adolescents. Methods： Twenty-five patients with subtrochanteric fractures were treated with the anatomical proximal femur bone plate. The mean operation time and blood loss were recorded, and the radiographic results and joint function were assessed. Results：The mean operation time was （55± 10） min, and mean blood loss was （50± 10） ml. All patients were followed up for 9 to 36 months postoperatively, meanly 25 months. No complications such as coxa vara, instruments failure, or infection were found. According to the Beatys standard, the early evaluation with good results was 96%, and the late evaluation with good results was 100% according to the Theologis＇s standard. For joint function, 24 patients were assessed as good, 1 patient as fair according to the Sanders＇s standard. The total percentage of good and fair was 96 %. Conclusion： For the treatment of subtrochanteric fractures in skeletally immature adolescents, the anatomical proximal femur bone plate has the advantages of minimal invasion, stable fixation, few complications and matching the adolescent characteristics of proximal femoral anatomy.
