肱骨近端骨折两种康复模式的比较研究

摘　　要：目的:比较"固定-理疗-活动"辩证统一康复模式与上臂固定带外固定康复模式对肱骨近端骨折的康复效果。方法:78例肱骨近端骨折住院病人,按照康复模式的不同随机分为"固定-理疗-活动"辩证统一康复模式组(简称"辩证组")和上臂固定带外固定组(简称"固定组"),前者48例,后者30例,以疼痛程度评分、肿胀程度评分、瘀斑情况评分、关节功能评分、血清肌酸激酶测定值、肩关节neer评分为观察指标,比较两组的短期、长期疗效。结果:短期观察,"辩证组"对疼痛程度评分、肿胀程度评分、血清肌酸激酶测定值的改善显著优于"固定组";长期观察,"辩证组"对肩关节neer评分的改善显著优于"固定组"。结论:"固定-理疗-活动"辩证统一康复模式对于肱骨近端骨折的康复效果优于单纯上臂固定带外固定。Objective：To compare the rehabilitation efficacy between the modality of dialectical unification of fixation, physi cal therapy, and activity, and the modality of external fixation by compression band in treatment of proximal humerus frac ture. Methods：All 78 inpatients with proximal humerus fracture were divided into two groups. 48 patients belong to the group of the modality of dialectical unification of fixation, physical therapy, and activity （abbreviating dialectical group）. 30 patients belong to the group of the modality of external fixation by compression band （abbreviating fixation group）. Compare the short and long term curative effect between the two groups by the indexes such as pain score, engorgement score, ecchymosis score, joint function score, and serum creatine kinase. Results： In short term, evaluated by the indexes such as pain score, engorgement score, and serum creatine kinase, the dialectical group notably outstrips the fixation group. In long term, evaluated by the shoulder joint neer score, the dialectical group notably outstrips the fixation group. Conclusion：The modality of dialectical unification of fixation, physical therapy, and activity, outstrips the modality of ex- ternal fixation by compression band.
