炎症状态与扩张型心肌病患者心功能和肾功能的关系

摘　　要：目的通过分析扩张型心肌病患者超敏C-反应蛋白(high sensitivity C-reactive protein,hs-CRP)与心功能和肾功能的关系,探讨炎症状态对患者的影响。方法纳入扩张型心肌病患者414例,将214例hs-CRP<3 mg/L的病例分为低炎症组,将200例hs-CRP≥3mg/L的病例分为高炎症组,比较两组心功能和肾功能的差异,并分析hs-CRP与心功能和肾功能的相关性。结果①高炎症组患者的心功能和肾功能均差于低炎症组,差异均有统计学意义;②hs-CRP与扩张型心肌病患者心功能相关性较好[左心室射血分数(LVEF):r=-0.26,左心室舒张末内径(LVEDD):r=0.35,N端脑钠肽前体(NT-proBNP):r=0.27,心功能分级(NYHA):r=0.47],与肾功能有一定的相关性[尿素氮(BUN):r=0.18,血肌酐(SCr):r=0.11,胱抑素C(CysC):r=0.15],差异均有统计学意义。结论高炎症状态严重影响扩张型心肌病患者的心功能和肾功能,炎症在扩张型心肌病病程中扮演着重要的作用。[Objective]To analyze the relationship between high sensitivity C reactive protein {hs-CRP} and cardiac function, renal function in patients with dilated cardiomyopathy, investigate the effect of inflmmnatory status on patients. [Methods]414 patients with dilated cardiomyopathy were divided into two groups, 214 cases whose hs-CRP level was less than 3 mg/L were the low-inflammatory group, and 200 cases whose hs-CRP level was equal or greater than 3 mg/L were the high-inflammatory group. The difference in cardiac function and renal function between two groups was compared, and the relationship between hs-CRP and cardiac function, renal function was analyzed. [ Results ] （1） The cardiac function and renal function of the high-inflanmmtory group were higher than those of the low-inflammatory group, and the differences were significant statistically. （2） The level of hs-CRP had good correlation with cardiac function of patients with dilated cardiomyopathy （ LVEF： r = -0.26; LVEDD r = 0.35 ; NT-proBNP： r = 0. 27; NYHA. r = 0.47 }, the level of hs-CRP had a cer- tain correlation with renal function [BUN： r = O. 18; SCr： r = 0. 11; CysC： r=0. 15）, and there were statistically significant. [ Conclusion] High inflammatory state seriously affected cardiac function and renal function in patients with dilated cardiomyopathy, and inflammation plays an important role in the course of dilated cardiomyopathy.
