不同浓度肝素盐水冲洗液对持续CVP监测患者凝血功能影响的比较研究

摘　　要：目的探讨不同浓度肝素盐水冲洗液对持续中心静脉压监测患者凝血功能的影响。方法将120例行持续中心静脉压监测的患者随机分为三组,A组冲洗液浓度为25U/ml,B组冲洗液浓度为12.5U/ml,C组冲洗液浓度为2.5U/ml;分别于患者持续监测中心静脉压24h、48h、72h后对监测患者的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、血浆凝血酶时间(TT)进行比较。结果A组患者在持续CVP监测后24h、48h、72h的PT、APTT、TT均较基础值显著延长(P<0.01),且时间越长和基础值偏离越大。B组患者24h、48h的PT、APTT、TT与基础值比较差异有显著意义(P<0.01),而72h后与基础值比较差异无显著意义(P>0.05)。C组患者24h、48h、72h的PT、APTT、TT与基础值比较差异均无显著意义(P>0.05)。结论对于持续CVP监测的患者,应采用2.5U/ml的肝素盐水作为冲洗液,并在持续CVP监测过程中,动态监测患者的凝血功能变化。Objective To study the impaction of heparin saline rinses with different density for continued CVP monitored patients＇ blood clotted. Method 120 patients with continued CVP monitoring are randomly divided into three groups, the rinses density of the group A is 25 U/ml, the rinses density of the group B is 12.5 U/ml, the rinses density of the group C is 2. 5 U/ml. The prothrombin time （PT）, activated partial thromboplastin time （APTT）, the thrombin time of plasma blood （TT） after 24 h,48 h,72 h of using the continued CVP monitoring was compared to monitor the skin and mucosa＇s hemorrhagic tendency, and get a quantitative score. Result The PT, APTT, TT in group A 24 h,48 h, 72 h after using the continued CVP monitoring was significant prolonged than that of the basic value（P〈0.01）, and the more longer time, the greater offset with the based value. There is significance difference for the PT, APTT, TT in group B in 24 h, 48 h compared with the basic value（P〈0.01）, but not for the 72 h（P〈0.05）. There is no significance difference for the PT, APTT, TT in group C in 24 h, 48 h, 72 h compared with the basic value （P〉0.05）. The score of the group A and B is obviously decrease with the extension of time. The hemorrhagic tendency was the latest in group C. There was significance difference between group A and C, and group B and C （P〈0. 05）. Conclusion For the continued CVP monitored patients, it should use the 2.5 U/ml heparin saline as rinses, and in the process of continued CVP monitoring, it should dynamically monitor the changes of patients＇ blood clotted function.
