抗双链DNA抗体及抗单链DNA抗体联合检测对系统性红斑狼疮早期诊断的价值

摘　　要：目的评价抗双链DNA抗体(antibody of double-strand DNA,anti-dsDNA)和抗单链DNA抗体(antibody of single-strand DNA,anti-ss DNA)单项及联合检测对系统性红斑狼疮(systemic lupus erythematosus,SLE)早期诊断的临床价值。方法选取某院风湿免疫科门诊和住院患者及健康体检者,用酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)法检测93例SLE患者、74例其他自身免疫性疾病患者和20例健康体检者血清中抗dsDNA抗体和抗ss-DNA抗体水平。结果 93例SLE患者血清自身抗体检测中,抗dsDNA抗体和抗ssDNA抗体的阳性率分别为72.0%和76.3%,特异性分别为89.2%和83.9%。两者联合检测,阳性率可提高到78.5%。随着SLE患者病情的发展和药物治疗的进行,血清中抗dsDNA抗体和抗ssDNA抗体水平同步变化。结论联合检测抗dsDNA抗体和抗ssDNA抗体可有效提高SLE的检出率,对于SLE的早期诊断、减少漏检和病情观察具有重要的临床意义。OBJECTIVE To analyze the early clinical diagnosis value of the single and combined detection for anti-dsDNA and anti-ssDNA antibody in systemic lupus erythematosus （SLE）. METHODS 93 cases of SLE, 74 cases of other autoimmune dis- eases and 20 healthy volunteers were detected by the method of enzyme-linked immunosorbent assay （ELISA）. RESULTS The serum positive rates of anti-dsDNA and anti-ssDNA antibody were 72.0% and 76.3% respectively, while the specificities were 89.2% and 83.9%. The sensibility of combined detection of the two indexes was 78.5%, which was higher than each of them. Based on the progress of SLE and therapy, both anti-dsDNA and anti-ssDNA antibody levels changed in the same trend. CONCLUSION Combination detection of anti-dsDNA and anti-ssDNA antibody could improve the positive detection rate of SLE, and is practical option for early diagnosis and monitoring the SLE progress.
