摘 要:目的探讨血清及诱导痰中白细胞介素8(IL-8)和白细胞介素17(IL-17)水平在老年慢性阻塞性肺疾病急性加重(AECOPD)评估中的价值。方法选取符合试验要求的129例慢性阻塞性肺疾病患者,使用简单随机化分组方法,将试验对象随机分为3组,每组各43人,测定诱导痰液和静脉血中的IL-8、IL-17水平。结果治疗前IL-8、治疗后IL-8痰上清液的浓度明显高于血清浓度,差异有统计意义(χ2值分别为56.731、49.828,P值均﹤0.001);治疗前IL-17、治疗后IL-17痰上清液的浓度明显低于血清浓度,差异有统计意义(χ2值分别为8.572、25.817,P值均﹤0.001)。治疗后痰IL-8及血IL-8的组间差异有统计学意义(χ2值分别为12.864、3.672;P值均﹤0.001)。治疗前好转患者血清IL-8和IL-17浓度明显低于非好转患者,且差异有统计学意义(Z值分别为2.714、1.873,P值均﹤0.05)。痰IL-17及血IL-8与痰IL-8呈正相关(β值分别为0.181、0.193,P值均﹤0.05)。而血IL-17与痰IL-8水平的相关性无明显统计学意义(β=0.034,P﹥0.05)。结论血清及痰IL-8和IL-17是评测AECOPD程度及预后的重要指标,应根据监测结果辩证用药,真正提高临床疗效。OBJECTIVE To explore serum and induced sputum leukocyte interleukin-8 (IL-8) and leukocyte interleukin 17 (IL-17) levels among elderly patients with acute exacerbation chronic obstructive pulmonary disease (AECOPD). METHODS One hundred and twenty-nine patients were randomly divided into three groups with 43 cases in each group. The serum and in- duced sputum IL-8, IL-17 level were assessed. RESULTS The sputum IL-8 concentrations before and after treatment were significantly higher than those of the serum IL-8. The differences were statistically significant ( X2 = 56.731, 49.828, P 〈 0.001 ). The sputum IL-17 concentrations before and after treatment were significantly lower than those of the serum IL-17. The differences were statistically significant ( X2 = 8.572, 25.817, P 〈 0.01). After treatment, the between-group differences be- tween sputum IL-8 and serum IL-8 were statistically significant (X2 = 12.864, 3.672; P 〈 0.05). Before treatment, the serum IL-8 and IL-17 concentrations of improved patients were significantly lower than those of non-improved patients. The differences were statistically significant (Z = 2.714, 1.873, P 〈 0.05). Sputum IL-17 and serum IL-8 were showed a positive correlation with sputum IL-8 (13 = 0.181, 0.193, P〈 0.05). However, the correlation of serum IL-17 and sputum IL-8 level was not sta- tistically significant (β = 0.034, P = 0.313). CONCLUSION IL-8 and IL-17 are important indicators for the evaluation of the severity and outcomes of AECOPD. It is recommended to treat patients according to the monitoring results to improve the clinical efficacy.
