不同剂量三聚氰胺染毒兔肾脏的CT灌注研究

摘  要:目的 探讨CT灌注成像对不同剂量三聚氰胺所致兔肾脏损伤的诊断价值。方法 新西兰大白兔36只,随机分为3组,每组12只。一组给予大剂量三聚氰胺灌胃,用量15g·kg^-1d^-1,连续2d,在第1次灌胃开始后2/1h、48h内分别进行肾脏CT灌注扫描;一组给予小剂量三聚氰胺连续灌胃60d,用量2.5g·kg^-1d^-1,每2周进行1次CT灌注扫描。12只不给予三聚氰胺,作为对照组,每2周进行1次CT灌注扫描。取双侧肾脏用外科显微镜观察后,制作石蜡切片(HE染色)观察肾脏病理改变及结石晶体形成情况。结果 大剂量组灌注的强化峰值明显下降,小剂量组肾脏的强化峰值无明显变化,二者强化峰值下降率之间的差异具有显著性意义(P〈0.01)。结论 大剂量三聚氰胺可导致肾脏急性微循环障碍。Objective To explore the value of CT perfusion imaging in evaluating the rabbit kidney injury caused by the different doses melamine. Methods Thirty-six New Zealand white rabbits were randomly divided into three groups equally. Ohe group re- ceived large doses of melamine gavage with the amount of 15 g·kg^-1d^-1 on two consecutive days. Two perfusion CT scan were performed respectively on kidney at 24, 48 hours after the gavage started. One group received small doses melamine orally for 60 days with the amount of 2.5 g·kg^-1d^-1. CT perfusion scan was performed every two weeks. The last 12 rabbits without any melamine were control group, which received CT perfusion scan every two weeks. The bilateral kidneys were first observed under surgical microscope, and then the histopathologic changes and crystal formation with paraffin sections(HE staining) were also observed. Results l.arge doses melamine group showed a remarkable decline of the value of the maximum enhancement peak, while the kidney strengthening peak of small doses group has no obvious change. The decline rates of enhancement peak between two groups differed significantly(P〈0. 01 ). Conclusion Large doses of melamine can cause kidney in acute microcirculatory disturbance.
