摘 要:目的研究山东济宁地区锌转运蛋白-8(SLC30A8)基因rs13266634单核苷酸多态性(SNP)的等位基因、基因型频率分布,探讨其与妊娠糖尿病(GDM)及相关代谢指标的关系。方法选取2010年10月至2012年3月济宁医学院附属医院产科门诊就诊的24~28周妊娠期妇女(451例)为研究对象,进行75g口服葡萄糖耐量试验(OGTT),其中203例诊断为妊娠糖尿病(GDM组),248名为妊娠糖耐量正常者(NGT组),应用聚合酶链反应一限制性片段长度多态性(PCR-RFLP)对203例GDM患者及248名妊娠糖耐量正常者进行基因分型,并进行生化指标检测,分别用胰岛13细胞分泌功能指数(HOMA-β)和稳态模型胰岛素抵抗指数(HOMA-IR)评估胰岛13细胞功能及胰岛素抵抗。两组间数据比较采用独立样本t检验,多组间比较采用方差分析,各组间基因型分布及等位基因频率比较采用X2检验,logistic回归分析基因与疾病的相关性。结果GDM组中CC基因型、C等位基因频率分别为42.9%(87/203)和65.5%(266/406),均显著高于NGT组的31.4%(78/248)和56.0%(278/496)(妒:6.259、8.362,均P〈0.05),而GDM组的TT基因型、T等位基因频率分别为11.8%(24/203)、34.5%(140/406),均显著低于NGT组的19.4%(48/248)、44.0%(218/496)(X2=4.721、8.362,均P〈0.05)。C等位基因携带者患GDM风险是T等位基因携带者的1.49倍(OR=1.490,95%CI为1.136~1.953,P:0.004)。CC基因型患GDM的风险是TT基因型的2.60倍(OR=2.600,95%CI为1.32l~5.119,P=0.006)。CC→CT→TT基因型间HOMA-β依次升高(分别为5.1±0.5→45.44±0.6→5.6±0.6,F=30.432,P〈0.01)。结论乩C30A8基因rs13266634SNP与山东济宁地区GDM有关,C等位基因可能是其风险等位基因,SLC30A8基因可能是该地区GDM易感基因之一。Objective To study the single nucleotide polymorphism of rs13266634 (the genotype and the frequency) in SLC30A8 gene and its association with Gestational Diabetes Mellitus and relevant metabolic parameters. Methods A total of 451 24 - 28 weeks of pregnant women who visited Affiliated Hospital of Jining Medical College from October 2010 to March 2012 were enrolled in this case-control study. All the participants underwent 75 g oral glucose tolerance test (OGTr) ,203 of them were diagnosed GDM (GDM group), and the others (248) were normal glucose tolerance (NGT group). The rs13266634 polymorphism in SLC30A8 gene was genotyped in 203 patients with Gestational Diabetes Mellitus and 248 pregnant women with normal glucose tolerance by PCR-RFLP analysis (polymerase chain reaction-restriction fragment length polymorphism). Meanwhile clinical characteristics such as body mass index(BMI), blood pressure( Bp), triglycerides (TG), total cholesterol ( TC), low density lipoprotein cholesterin ( LDL-C), high density lipoprotein cholesterin (HDL-C), etc were measured and calculated. HOMA^3 and HOMA-IR were used to estimate 13-cell function and insulin resistance. We used t-testing of independent samples to compare the difference between two groups and used analysis of variance to test the difference amonz theseveral groups, X2analysis was used to compare the difference in genotype and allele frequencies. Logistic regression analysis was used to test for association between rs13266634 polymorphism in SLC30A8 gene and GDM.
