摘　　要：目的探讨母乳性黄疸婴儿免疫功能水平的变化。方法母乳性黄疸患儿53例,按血清总胆红素(totalbilirubin,TBIL)水平分为重度组(TBIL≥342μmol/L)12例、中度组(256μmol/L<TBIL<342μmol/L)20例与轻度组(TBIL≤256μmol/L)21例,选取同期母乳喂养健康新生婴儿18例为对照组,测定4组血清IgG,IgA,IgM,C3及C4含量。结果重度组IgG,IgA,IgM,C3及C4水平均低于中度组、轻度组与对照组(P<0.05);中度组IgG,C3及C4水平低于轻度组与对照组(P<0.05),IgA及IgM水平与轻度组和对照组比较差异无统计学意义(P>0.05);轻度组IgG,IgA,IgM,C3及C4水平与对照组比较差异无统计学意义(P>0.05);重度组治疗后IgG,IgA,IgM,C3及C4水平均高于治疗前,差异有统计学意义(P<0.05)。结论母乳性黄疸患儿TBIL水平≥342μmol/L时,免疫功能低下,早期干预可短期恢复免疫损伤。Objective To explore the immune function in infants with breast milk jaundice. Methods Fifty-three infants with breast milk jaundice were divided into severe group （n= 12, total bilirubin≥342 μmol/L）, moderate group （n= 20, total bilirubin： 256 to 342μmol/L）, and mild group （n= 21, total bilirubin≤256 μmol/L）. Another 18 health breastfeeding infants were as control group. The serum levels of IgG, IgA, IgM, C3 and C4 were detected. Results The levels of IgG, IgA, IgM, C3 and C4 were lower in severe group than those in moderate group, mild group and control group （P〈0.05）. The levels of IgG, Ca and C4 were lower in moderate group than those in mild group and control group （P〈0.05）. There were no significant differences in IgA and lgM in moderate group in comparison with mild group and control group （P〉0.05）. There were no significant differences in IgO, IgA, IgM, C3 and C4 levels between mild group and control group （P〉0.05）. And all the above indexes were higher after treatment than those before treatment in severe group （P〈0.05）. Conclusion When serum total bilirubin level is 342μmol/L or higher in infants with breast milk jaundice, the immune function is lower. Therefore early intervention could improve immune function in short period.
