经皮微创和Henry入路掌侧锁定钢板治疗桡骨远端骨折的疗效比较

摘　　要：目的比较微创经皮钢板接骨术（minimalinvasivepereutaneousplateosteosynthesis，MIPPO）和常规前臂Henry入路锁定钢板治疗桡骨远端骨折的疗效。方法回顾性分析应用MIPPO技术和常规前臂Henry入路掌侧锁定钢板治疗桡骨远端骨折患者38例，按随机数字表法分为MIPPO组（20例）和Henry组（18例）。比较两组手术时间、术中出血量、并发症、骨折愈合时间、腕关节主动活动范围及上肢功能评定标准（disabilitiesofthearill，shoulderandhand，DASH）评分。结果术后随访12～18个月，平均15．5个月。按照DASH评分：MIPPO组（6．16±2．45）分，优于Henry组的（10．94±4．25）分（P〈0．05）。MIPPO组手术时间较Henry组短（P〈0．05），术中出血量较Henry组少（P〈0．05）。两组在骨折愈合时间、并发症、腕关节主动活动范围方面差异均无统计学意义。结论MIPPO和Henry人路掌侧锁定钢板均能有效治疗桡骨远端骨折。Objective To compare clinical effects of minimal iuvasive percutaneous plate osteosynthesis （MIPPO） and ordinary forearm Henry approach palmar locking plate in treatment of distal radial fractures. Methods A retrospective analysis was conducted on the 38 patients with distal radius fracture treated by MIPPO technique and ordinary Henry approach palmar locking plate. The patients were randomly divided into MIPPO group （n = 20） and Henry group （n = 18）. The two groups were compared in aspects of operation time, intraoperative blood loss, complications, fracture healing time, active range of motion of wrist joint and disabilities of the aim, shoulder and hand （DASH） score. Results Duration of follow-up was 12 to 18 months （ average 15.5 months）. DASH score in MIPPO group [ （6.16±2.45） points] was better than that in Henry group [ （ 10.94 ±4.25） points] （P 〈0.05）. MIPPO group showed a shorter operation time （P 〈 0.05） and less intraoperative blood loss （P 〈0.05 ） than Henry group. However, there were no significant differences between the two groups regarding fracture healing time, complications, active movement range of wrist joint. Conclusion MIPPO technique and Henry approach palmar locking plate are both effective in treatment of distal radial fractures.
