摘  要:目的:探讨羊水Ⅲ度粪染(胎儿宫内窘迫)与脐动脉血pH值的关系及其对于新生儿胎儿宫内缺氧的早期诊断、预后的意义。方法:从2011年3月~2011年10月我院剖宫产的新生儿,抽取羊水Ⅲ度粪染的新生儿95例,术中采脐血pH值,作为研究组(A组)。随机抽取100例羊水清,新生儿正常,作为对照组(B组)。对两组脐血血气pH值及Apgar评分进行分析,并对结果进行比较。结果:新生儿窒息的发生随脐血pH值降低而升高,正常评分的发生率随pH值降低而减少,两者呈显著性差异(P小于0.01)。结论:脐血pH值是反映胎儿组织代谢状态的最高指标,用来判断酸碱异常的类型和严重程度。Objectives: To investigate the relationship between Grade III meconium -stained amniotic fluid (fetal distress) and the pH value of umbilical artery blood. This research is of great significance in early diagnosis and prognosis. Method: We extracted grade Ⅲ meconium stained amniotic fluid from 95 eases meconium stained newborns in our hospital from March 2011 to October 2011 and collect pH values of these neonates' cord (A group). 100 cases of clear amniotic fluid samples were randomly selected from normal newborns as the control group (B). Then we compared the blood gas indexes of card blood and pH values in these two groups and analyze the date with Apgar marking system. Results: Neonatal asphyxia increased while the cord blood pH value decreased. The rate of normal scoring increased while the cord blood pH values decreased, indicating a significant differ- ence ( P less than 0. 01 ). Conclusion: The pH value of cord blood reflects the highest index of the metabolic state of fetal tissue and can be used to judge the type of acid - base imbalance and to evaluate the severity.
