NF-κB阻滞剂PDTC在冠脉搭桥术中对心肌的保护效果

摘　　要：目的:探讨心脏手术过程中NF-κB阻滞剂对心肌的影响。方法:将20例同期拟行冠脉搭桥术(CABG)患者分为对照组和观察组各10例,2组麻醉方式相同,均采用体外循环下停跳搭桥术,术中分别用含NF-κB阻滞剂PDTC(观察组)和普通的冷晶体停搏液(对照组)作为心肌保护液。2组分别于术前1h、术中、术后1h、术后24h测定患者血清肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白I(cTnI)、乳酸脱氢酶(LDH)水平。结果:2组术前1h血清CK-MB、cTnI和LDH水平差异无统计学意义,而术中、术后2组血清CK-MB、cTnI、LDH均升高,但观察组3个指标的水平均低于对照组(F组间分别为121.003、32.326和250.088,F时间分别为53.802、51.574和227.901,P均<0.05)。结论:NF-κB阻滞剂可有效减轻CABG患者的心肌损伤。Aim：To observe the protective effect of the NF-κB retardant on myocardium during coronary artery bypass grafting（CABG）.Methods：A total of 20 patients undergoing CABG were allocated into two groups.Group A was given cold crystal cardioplegia containing the NF-κB retardant PDTC,and group B used the ordinary cold crystal cardioplegia.The 2 groups were given the same narcosis and operation way.Before the operation,during the operation,1 h and 24 h after the operation,the serum levels of creatine kinase（CK-MB）,myocardium troponin（cTnI） and lactic dehydrogenase（LDH） were determined.Results：There were no statistic differences in the indexes mentioned above between the 2 groups before the operation（P0.05）.The levels of CK-MB,cTnI and LDH of the 2 groups were higher during the operation,1 h and 24 h after the operation than before the operation,but those of group A were lower than group B（Fgroup=121.003,32.326 and 250.088;Ftime=53.802,51.574 and 227.901;P0.05）.Conclusion：The NF-κB retardant could reduce the injury to myocardium during CABG.
