MSM人群中艾滋病病人服药依从性的心理状况分析及其护理

摘　　要：目的探讨心理护理对男男性行为人群(Men who have sex with men,MSM)中艾滋病病人(MSM艾滋病病人)抗病毒治疗依从性的影响。方法选取200例MSM艾滋病病人,在服用抗病毒药物的同时,由副主任医师、副主任护师和心理专家一起制定心理护理方案,实施综合心理护理。由专职护士定期收集病人服药依从性信息,并进行心理状况评测。结果实施心理护理后,在恐惧不安、害怕不良反应、焦虑冲动、沮丧抑郁和怨恨报复方面有显著改善。按医嘱定时服药的比例从89.8%增加至96.9%,干预前后差异有统计学意义(P=0.004);服药剂量的正确率从91.8%增加至99.0%,差异有统计学意义(P=0.001);漏服率在5%以下者所占比例从90.3%增加至97.5%,差异有统计学意义(P=0.003)。结论心理护理能够改善病人的心理状态,增加艾滋病治疗的依从性。Objective To study the psychological care to AIDS patients among men who have sex with men（MSM） and its impact on their adherence to in antiretroviral treatment（ART）.Methods Two hundred cases of MSM living with AIDS were enrolled.While taking antiviral drugs,the patients received a comprehensive psychological care scheme prepared by the associate chief physician,associate chief nurse and psychologist.Information on medication adherence was collected regularly by nurses and the psychological status were evaluated.Results The status of fear,upset,being afraid of side effects,anxiety,impulse,frustration,depression and resentment,revenge had significantly improved.Proportion of people taking medicine timely by following the prescriptions significantly increased from 89.8% to 96.9% after intervention（P= 0.004）;proportion of correct dosage increased from 91.8% to 99.0%,and the difference had statistical significance（P= 0.001）;and the proportion of patients with 5% or less medication missing increased from 90.3% to 97.5%（P= 0.003）.Conclusion The psychological care can improve psychological status,and increase adherence to the treatment of AIDS.
