气压治疗和CPM锻炼预防髋部术后下肢深静脉血栓形成的临床观察

摘　　要：目的:观察气压治疗和CPM锻炼预防骨科髋部大手术后下肢深静脉血栓形成(DVT)的临床效果。方法:我院2009年7月-2012年2月在基本预防措施上加用肢体气压治疗和CPM锻炼预防DVT的133例髋部手术病例为观察组,2006年7月-2009年6月间采取基本预防措施(患肢抬高、肌肉舒缩锻炼及关节主被动活动、按摩)预防DVT的127例髋部手术病例为对照组。回顾性分析两组的临床疗效、术后引流量、疼痛、大腿肿胀变化、下肢血管彩色多普勒检查。结果:两组在临床疗效、术后引流量、疼痛等方面差异无统计学意义(P>0.05),两组大腿肿胀变化差异及术后下肢DVT发生率有统计学意义(P<0.05),观察组未见明显不良反应。结论:基本预防措施上加用肢体气压治疗和CPM锻炼预防髋部大手术后下肢深静脉血栓形成临床疗效优于单纯基本预防措施。Objective：To observe the clinical effect and application value of preventing crural deep venous thrombosis（DVT） by Intermittent pneumatic compression（IPC）and CPM after orthopedic surgery of hip.Methods：By reviewing 236 patients of hip fractures managed with surgically or Hip Arthroplasty during July 2007 to June 2012,The patients（n=103） as matched groud taken Basic preventive measures from July 2007 to June 2009,such as Limb elevation,muscle vasomotor exercise,joint passive activity,massage,etal.and others（n=133） as control group,who taken IPC,CPM and Basic preventive measures in July 2009 to June 2012,were retrospectively analysis the curative effect,incision drainage,Pain,thigh swelling changes,shows of venous by color doppler ultrasonography.Results：The control group in the clinical effect,thigh swelling changes,was better than matched group,especially in improving clinical effect.The rate of DVT was statistically significant in two groups（P0.05）,There were no adverse reaction observed in the control group.Conclusion：Taken IPC,CPM and Basic preventive measures can be safe and effective to prevent the crural deep venous thrombosis after hip operation,which better than taken Basic preventive measures.
