摘  要:目的探讨地方性氟中毒患者T淋巴细胞及Thl与Th2细胞因子的变化对机体免疫机制的影响,为临床治疗Thl/Th2失衡提供理论依据。方法采用病例一对照方法分析研究对象,选取甘肃省会宁县2011年确诊地方性氟中毒患者20例(均居住于水氟高于1.0mg/L的高氟区,患有氟斑牙或氟骨症)为实验组,其中男12例,女8例,年龄45~76岁;同时选取兰州大学第一医院体检中心2011年5月健康体检者20例为对照组,其中男10例,女lO例,年龄40~65岁。患者与正常对照者均抽取空腹静脉血3ml,使用流式细胞仪检测外周血T淋巴细胞亚群,同时用双抗夹心酶联免疫吸附测定法(ELISA—sandwichtechnique)检测血清中Thl细胞因子、Th2细胞因子的水平。结果外周血T细胞亚群CD3+细胞的含量在氟中毒患者组和对照组之间差异无统计学意义,CD4十淋巴细胞含量较对照组明显下降(P〈0.05),CD8+淋巴细胞含量较对照组明显升高(P〈O.05),CD4+/CD8+比值较对照组明显下降(P〈0.05);氟中毒患者血清中Thl细胞因子IFN-7的水平明显高于对照组,差异具有统计学意义(P〈0.05);Th2细胞因子IL-4的水平明显低于对照组,差异具有统计学意义(P〈0.05)。结论氟中毒患者体内存在着T淋巴细胞亚群及细胞因子失衡的现象,通过研究氟中毒患者外周血中Thi/Th2细胞因子状态,抑制CD4+T细胞和其活化,有效抑制Thl前炎性细胞因子,恢复患者体内Thl/Th2平衡,达到治疗的目的。Objective To study the T lymphocytes and Thl,Th2 cytokine changes in patients with endemic fluorosis on the immune mechanisms,analysis of disease occurrence and development of the situation and provide the correct treatment for clinical adjustment of Thl/Th2 imbalance. Methods T lymphocyte subsets in peripheral blood of patients with endemic flu- orosis were determined and analysed by flow cytometry, double-antibody sandwich enzyme-linked immunosorbent assay were used to detect serum Thl cytokines,Th2 cytokine levels in patients with endemic fluorosis. Results Fluorosis patients and normal peripheral blood T lymphocytes CD3 + no significant changes, CD4+ lymphocytes decreased,CDS+ lymphocytes significantly increased CD4+/CDS+ ratio was significantly lower. Thl cytokine IFN-r levels were significantly higher and Th2 cytokines IL-4 levels were significantly lower than the control group. Conclusion T lymphocyte subsets and cytokine imbalance were found in fluorosis patients peripheral blood. Through research the Thl/Th2 cytokine status, inhibition of CD4+ T cells and its activation, the effective inhibition of Thl proinflammatory cytokines, restoring the patient's body Thl/ Th2 balance to achieve the purpose of treatment.
