摘 要:目的探讨运用术前及术中MRI进行垂体腺瘤神经导航手术的作用。方法对44例垂体腺瘤患者于术前在导航系统工作站中进行MRI三维重建,制定出手术计划,标记重要结构。术中应用MRI导航定位并指导手术操作。结果所有MRI导航手术均获成功,术中神经导航定位精确,注册误差0-1.9mm。术后MRI复查肿瘤全切除者36例(81.9%)、次全切除者8例(18.1%)。结论神经外科手术导航系统是借助医学影像提供的信息提高手术的精度和安全性的一种技术手段;MRI是进行垂体腺瘤导航手术最适合的导航基础图像。Objective To investigate the role of preoperative and intraoperative MR image in neuronavigation system-guided transsphenoidal resection of pituitary adenoma. Methods MR scaning was performed in 44 patients before surgery.Images reconstruction,designing the operation plan and marking important structures were carried out in the workstation of navigation system.The operation procedures were done under the navigation system guided by MR image. Results With neuronavi-gation system,all the image-guided operations were successful with accurately localizing tumor and a mean error of 0-1.9mm.Total resection was performed in 36(81.9%) cases and subtotal resection in 8(18.1%) cases confirmed by postoperative MR image. Conclusion Neurosurgery navigation system increases the accuracy and safety of surgery because of the visualization supporting of imaging information.MR image is the best guiding-image of neuronavigation system.
