改良胃镜清洗消毒方法对防止医源性乙型肝炎病毒交叉感染的效果

摘　　要：目的探讨改良胃镜清洗消毒方法对防止医源性乙型肝炎病毒(HBV)交叉感染的效果。方法采用常规及改良消毒法监测4个项目,取样50次/项目,进行分组清洗消毒;改良胃镜清洗消毒方法是在常规消毒方法的基础上增加了增压泵及清洗喷枪,以取代常规方法中使用的注射器抽吸灌洗;对比分析两种消毒方法的效果。结果改良胃镜清洗消毒方法的胃镜合格率为100.00%,明显优于常规消毒法的76.00%,差异有统计学意义(P<0.05);且改良消毒组的活检钳的灭菌效果、2%戊二醛及消毒液的有效浓度合格率分别为96.00%、96.00%、98.00%,常规消毒法的合格率分别为72.00%、80.00%、80.00%,两种消毒方法差异有统计学意义(P<0.05)。结论改良胃镜清洗消毒方法能够有效地防止医源性HBV交叉感染,且成本低、实用性强,值得临床推广。OBJECTIVE To explore the modified cleaning and disinfection method of gastroscopes so as to prevent the iatrogenic cross infections caused by hepatitis B virus.METHODS Totally 4 items were monitored by the conventional disinfection method and the modified disinfection method and were sampled for 50 times per item,then being disinfected by dividing them into groups.Based on the routine manual cleaning and sterilization,a micro pump and a sprayer were used to replace the routine injector of 50 ml to douche the gastroscopes for the purpose of preventing cross infection of HBV.The effects of the two disinfection method were compared and analyzed.RESULTS The qualified rate of the gastroscopes by the modified cleaning and disinfection method was 100.00%,which was significantly higher than 76.00%,the difference was statistically significant（P0.05）.The qualified rates of the biopsy forceps（96.00%）,the effective concentration of 2% glutaraldehyde（96.00%）and disinfectant（98.00%）of the modified disinfection group were significantly higher than those of the conventional disinfection group（72.00%,80.00%,and 80.00%）,the difference between the two disinfection methods were statistically significant（P0.05）.CONCLUSION The modified cleaning and disinfection method,with less cost and more practical use,can effectively prevent the iatrogenic cross infections caused by HBV,which is worthy of the promotion in the hospital.
