胃镜室感染因素分析及控制对策

摘　　要：目的分析胃镜室的感染因素,制定控制胃镜室感染的相应措施,以降低感染率。方法对比医院胃镜室布局改进、医护人员洗手、电脑键盘消毒前后及胃镜正确消毒的细菌检测结果,制定出有效减少胃镜室感染的措施。结果 2002年改进后胃镜清洗与消毒室内空气细菌检测合格率为71.05%,改进前为35.71%;医护人员洗手后及键盘定期消毒后细菌检测合格率分别为92.11%及94.74%,医护人员洗手前及键盘定期消毒前细菌检测合格率分别为52.63%、39.47%;胃镜经完全浸泡的细菌数为(6.2±2.1)CFU/件,胃镜部分浸泡的细菌数为(8.8±3.2)CFU/件,差异均有统计学意义(P<0.05)。结论通过对胃镜室的科学布局,规范医护人员的日常工作规范,注意洗手和定期对电脑键盘的消毒,正确清洗、消毒胃镜能有效减少胃镜室感染的发生。OBJECTIVE To analyze the factors of infections in the gastroscope room and to summarize the corresponding measures to control the nosocomial infections in the gastroscope room so as to reduce the infection rate.METHODS The detection of bacteria was observed before and after the improvement of layout of the gastroscope room,the hand washing of the medical staff,the disinfection of the computer keyboard,and the correct disinfection of gastroscope.The effective measures were put forward to reduce the infections in the gastroscope room.RESULTS The qualified rate of bacterial detection of collected air samples and the gastroscope cleaning was 71.05% after the improvement in 2002,35.71% before the improvement;the qualified rate of the bacterial detection was 92.11% after the hand washing of the medical personnel and 52.63% before the hand washing,while the qualified rate of the bacterial detection was 94.74% after the regular disinfection of the computer keyboard and 39.47% before the regular disinfection of the computer keyboard.The bacterial colony count was（6.2±2.1） CFU/piece through complete immersion of the gastroscope and（8.8±3.2） CFU/piece through partial immersion of the gastroscope,the differences were all statistically significant（P0.05）.CONCLUSION It is an effective way to reduce the incidence of infections in the gastroscope room through the scientific layout of the gastroscope room,standardized daily work of medical staff,hand washing,regular disinfection of the computer keyboard,and correct cleaning and disinfection of the gastroscopes.
