摘 要:目的针对VITEK-2Compact在检测鲍氏不动杆菌对阿米卡星药敏试验中的局限性,利用金标准E-test法探索不同耐药表型和检测结果的相关性,为临床提供更加准确的结果。方法运用VITEK-2Compact AST-GN13药敏卡测定296株鲍氏不动杆菌的药敏试验结果,按耐药表型将所测菌株分为3组即1组仅阿米卡星敏感、2组仅阿米卡星及亚胺培南敏感、3组除阿米卡星及亚胺培南外存在其他被检测药物敏感;每组随机抽取40株用E-test法作为金标准同时进行K-B法测定鲍氏不动杆菌对阿米卡星的敏感性。结果 3组检测鲍氏不动杆菌K-B法和E-test法均一致;1组VITEK-2Compact结果与K-B法及E-test法结果不一致,2组39株不一致、1株一致,3组2株结果不一致、38株结果一致;同时观察到VITEK-2Compact AES专家系统显示耐药表型为野生株的29株3种方法结果一致。结论试验结果证实K-B法及E-test法的结果是一致的,所以VITEK-2Compact用户可以采用K-B法检测鲍氏不动杆菌对阿米卡星的敏感性。OBJECTIVE To discuss the limitations of VITEK-2 Compact in the detection of the drug susceptibility of Acinetobacter baumannii to amikacin and explore the relationship between the drug resistant phenotypes and the detection result with gold standards E-test so as to offer the hospital more accurate statistics.METHODS The drug susceptibility testing results of 296 strains of A.baumannii were tested with VITEK-2 Compact AST-GN13.All the tested strains were divided into three groups according to the drug resistant phenotypes,the strains in the group A were only sensitive to amikacin,the strains in the group B were sensitive to amikacin and imipenem,and the strains in the groupc were sensitive to all the tested antibiotics except amikacin and imipenem.Each group was randomly extracted 40 strains,by the use of E-test as the gold standards,the drug susceptibility of the A.baumannii strains to amikacin was determined with K-B method.RESULTS The E-test method and the Kirby-Bauer method were consistent in the detection of A.baumannii.In the group A,the results of VITEK-2 compact differed from the results of the K-B method and E-test;in the group B,there were 39 strains being not consistent and 1 strain being consistent;in group C,there were 2 strains being not consistent and 38 strains being consistent.VITEK-2 Compact AES indicated that the results of the three methods were consistent to the 29 wild strains as the drug resistant phenotypes.CONCLUSION The study concludes that it is consistent to the results of the E-test and K-B method,thus the VITEK-2 Compact users can adopt the K-B method to detect the susceptibility of A.baumannii to amikacin.
