泛耐药鲍曼不动杆菌中发现β-内酰胺酶基因新的变异型ADC-65、ADC-66

摘　　要：目的调查一组泛耐药鲍曼不动杆菌(PDR-ABA)中β-内酰胺酶基因存在和变异情况。方法收集2010年7月至2010年12月某医院住院患者之痰液标本中分离的泛耐药鲍曼不动杆菌共20株,用分子鉴定法鉴定菌种,再用聚合酶链反应(PCR)及序列分析的方法分析A类～D类共34种β-内酰胺酶基因。结果本组20株泛耐药鲍曼不动杆菌共检出TEM-1型20株(100.0%)、PER-1型16株(80.0%)、OXA-23型20株(100.0%)、OXA-66型20株(100.0%)、ADC-30型16株(80.0%)、ADC基因新的变异型ADC-65型3株(15.0%)(GenBank登录号:JX109941)、ADC-66型1株(5.0%)(GenBank登录号:JX109942)。结论本组鲍曼不动杆菌对β-内酰胺类药物耐药主要与产TEM-1、PER-1、OXA-23型、OXA-66型、ADC等β-内酰胺酶基因相关,OXA-23型β-内酰胺酶基因阳性是泛耐药菌耐碳青霉烯类药物的原因。发现ADC基因存在新变异型:ADC-65、ADC-66是国内外首次报道。Objective To investigate the distribution and variety of β--1actamase genes in a group of pandrug- resistant Acinetobacter baumannii. Methods From July 2010 to December 2010, 20 strains of pandrug-resistant Acinetobacter baumannii were collected from the sputum samples of patients in a hospital in China. Then, strains were identified by molecule identification, and 34 kinds of β--1actamase genes were analyzed by polymerase chain reaction（PCR） and DNA sequencing. Results In 20 strains of Acinetobacter baumannii, 20 strains of TEM-1 （100.0%）, 16 strains of PER-1 （80.0%）, 20 strains of OXA-23（100.0%）, 20 strains of OXA-66（100.0%）, 16 strains of ADC-30 （80.0%）, 3 strains of novel variants of ADC： ADC-65 （15.0%）（GenBank accession number： JX109941）, 1 strains of novel variants of ADC： ADC-66 （5.0%）（GenBank accession number： JX 109942） were positive. Conclusion In this group of Acinetobacter baumannii, producing β--1actamase genes TEM-1, PER-l, OXA-23, OXA-66, ADC, played a key role in resistance to β--1actams. And OXA-23 contributes to resistance to carbapenems in pandrug- resistant strains. And it＇s the first report that novel variant of ADC genes： ADC-65 and ADC-66, were discovered in the world.
