复方利多卡因乳膏在小儿无痛静脉穿刺中的作用

摘　　要：目的探讨5%复方利多卡因乳膏在小儿无痛静脉穿刺中应用及效果分析。方法随机选取住院患儿160例随机分为干预组和对照组。干预组在以穿刺点为圆心的局部皮肤上,涂上0.5g/5cm2的5%复方利多卡因乳膏并用无菌透明敷贴覆盖1h,去除敷贴和乳膏后行静脉穿刺术;对照组行常规静脉穿刺。两组均由专人观察记录患儿的面部疼痛表情评分、肢体抵抗情况、自诉痛觉评价,穿刺成功率,父母对5%复方利多卡因乳膏的评价。结果干预组的面部疼痛表情评分(χ2=80)、肢体抵抗情况(χ2=174.72)、自诉痛觉评价(χ2=136),穿刺成功率(χ2=9.61),父母对5%复方利多卡因乳膏的评价(χ2=70.32)明显高于对照组,P<0.05,差异具有统计学意义。结论 5%复方利多卡因乳膏在小儿静脉穿刺中的镇痛作用好,效果显著,宜在儿科临床推广使用。Objective To discuss the clinical using and effect of 5% compound lidocaine ointment cream in peadiatrics painless venipuncture.Methods To select one hundred and sixty inpatient children sickening for venipuncture and to divide them randomly into intervention group and control group.To smear 0.5g/5cm2 compound lidocaine ointment on the skin round the venipunture point of the sick children in intervention group and to cover a sterile and transparent dressing for about one hour,and then to conduct venipuncture,after removing the sterile dressing and cream.To routine peadiatrics venipuncture for control group.The both group to arrange professional people to observe and record the children’s painful expression,extremities’ resistance,complaint of pains,rate of venipunctme success,the parents’ judgement for 5% compound lidocaine ointment cream,and so on.Results At the children’s painful expression（χ2=80）,extremities’ resistance（χ2=174.72）,complaint of pains（χ2=136）,rate of venipunctme success（χ2= 9.61）,the parents’ judgement for 5% compound lidocaine ointment cream（χ2=70.32）,children of intervention group feel significantly less painful than them of control group,P0.05 and the difference has statistics’ significance.Conclusion 5% compound lidocaine ointment cream has good effect in relieving pain in peadiatrics venipunctme.The effect is prominent.It is suitable to use compound lidocaine ointment cream in peadiatrics clinical treatment.
