不同麻醉方法对小儿气管异物取出术麻醉效果和苏醒质量的影响

摘　　要：目的探讨七氟醚＋舒芬太尼＋丙泊酚＋阿曲库铵并高频射通（HFJV）在小儿气管异物取出术中的效果及安全性。方法30例小儿气管异物取出术随机分为七氟醚＋舒芬太尼＋丙泊酚＋阿曲库铵麻醉组（A组）和氯胺酮＋瑞芬太尼＋丙泊酚＋阿曲库铵（B组），每组15例。．比较两组患儿手术时间、苏醒时间、术中术后并发症及置镜容易度。结果A组手术时间明显短于B组（P〈0．05），B组苏醒时间短于A组（P〉0．05），A组置镜容易度优于B组（P〈0．05），A组苏醒期不良反应发生率明显少于B组（P〈0．05）。结论小儿气管异物取出术应用七氟醚＋舒芬太尼＋丙泊酚＋阿曲库铵麻醉效果及复苏质量优于氯胺酮＋瑞芬太尼＋丙泊酚＋阿曲库铵静脉麻醉。Objective To investigate the efficacy and safety of sevoflurane ＋ sufentanil ＋ propo- fol ＋ atracurium and high-frequency radio-pass （HFJV） on pediatric tracheal foreign body removal surger- y. Methods Thirty cases of pediatric tracheal foreign body removal surgery were randomly divided into sevoflurane ＋ sufentanil ＋ propofol ＋ atracurium anesthesia group （ group A） and ketamine ＋ remifentanil ＋ propofol ＋ atracurium group （ group B ）, 15 cases in each group. To compare operative time, recovery time, postoperative complications and set mirror ease between the two groups. Results Operative time in group A was significantly shorter than in group B （P 〈 0.05）, group B regained consciousness time was shorter than that in group A （P 〉 O. 05 ）, group A set mirror easily than group B （P 〈 0.05 ）. Incidence of adverse reactions of group A in wake period was significantly less compared with group B （ P 〈 0.05 ）. Conclusions Anesthetic effect and recovery quality of pediatric tracheobronchial foreign body removal surgery use sevoflurane ＋ sufentanil ＋ propofol ＋ atraeurium ammonium is better than ketamine ＋ remifen- tanil ＋ propofol ＋ atracurium intravenous anesthesia.
