彩色多普勒和超声能量图在乳腺癌新辅助化疗前后诊断中的价值

摘　　要：目的探讨彩色多普勒和超声能量图在乳腺癌新辅助化疗前后诊断中的价值。方法选择2011年3月至2012年3月住院的85例乳腺癌患者,化疗前采用彩色多普勒和超声能量图对病灶、淋巴结大小、血流动力学及血流分布等进行检查,采用多西他赛+表柔比星(TE)方案化疗,比较化疗前后各项指标变化情况。结果 85例患者经3个周期化疗后,9例患者症状完全缓解,其余76例患者病灶体积较治疗前明显缩小,边界、形态、周边强回声较治疗前有显著变化,治疗前后比较差异有统计学意义(P<0.05)。化疗前血流分级绝大多数是3级,无0级和1级,化疗后分级有显著变化,3级病例数大大减少,与化疗前比较差异有统计学意义(P<0.05)。结论彩色多普勒和超声能量图联合应用,可对比乳腺癌新辅助化疗前后病灶变化,对判断化疗药物的敏感性提供指导,且检查方法安全、简单、方便。Objective to study the diagnostic value of color doppler ultrasound energy figure in breast cancer before and after neoadjuvant chemotherapy.Methods Choose 85 breast cancer patients from 2011 March to 2012 March,before chemotherapy using color Doppler ultrasound energy and diagram of the lesion,lymph node size,hemodynamics and blood flow distribution check,using docetaxel and epirubicin（TE） regimen of chemotherapy,compare the changes of indexes before and after chemotherapy.Results 85 cases of 3 cycles of chemotherapy,9 patients completely ease symptoms,the rest of the 76 patients lesions is decrease volume before treatment,the boundary,shape,and the peripheral strong echo is a significant change before treatment,compared before and after treatment with significant difference,there is statistical significance（P0.05）.before chemotherapy most is 3 blood flow grade level,no zero and 1 level,after chemotherapy classification has changed dramatically,grade 3 cases decrease greatly,there is statistical significance（P0.05）.Conclusions Color doppler ultrasound energy figure can compare lesions change of breast cancer before and after neoadjuvant chemotherapy,to judge the chemotherapy drug sensitivity and providing guidance,and the inspection method is safety,simple and convenient.
