摘 要:目的检测震荡排痰器对人体呼吸道分泌物清除的影响。方法采用自身对照的方法,16名健康受试者吸入等剂量的同位素标记的气溶胶,分别测试自然呼吸和使用震荡排痰器情况下标记气溶胶的半数清除时间,观察其对机体呼吸道分泌物清除的影响。结果震荡排痰器可明显缩短主气道44.40±7.13 vs 60.20±5.87 min,(P<0.01)、肺门50.98±4.22 vs 67.83±3.36 min,(P<0.01)和肺尖26.95±3.89 vs 31.18±2.09 min,(P<0.01)等部位的同位素标记气溶胶的半数清除时间;干预组(F=88.412,P<0.01)与对照组(F=357.925,P<0.01)不同观察部位的放射性气溶胶半数清除时间存在差异;干预组与对照组的主气道、肺门、肺尖的分泌物加快清除率存在差异(F=10.726,P<0.001),且不同部位的显效比率亦存在差异(x2=8.400,P=0.015);FEV1/FVC%(81.6±9.2)%vs(82.9±8.7)%,(P>0.05)和PEF值318.7±56.2 ml vs 305.3±69.3 ml,(P>0.05)均无明显改变。结论震荡排痰对呼吸道分泌物的清除效果确切、使用安全,是一种实用的物理排痰方法。Objective To evaluate the application of a flutter mucus-clearance device in clearance of airway secretions.Methods Sixteen healthy volunteers were enrolled and self-control design was used in the study.Isotope-labeled aerosol was inhaled,half-clearing time of isotopic aerosol was detected with natural breathing and with the flutter mucus-clearance device,respectively.The efficacy of the device in clearance of airway secretions was observed.Results Compared to self control,the half-clearing time of isotopic aerosol in the device using was reduced significantly in the main airway44.40±7.13 vs 60.20±5.87 min,(P0.01),hilum 50.98±4.22 vs 67.83±3.36min,(P0.01) and apex 26.95±3.89 vs 31.18±2.09min,(P0.01) of the lung.Among different parts of the lung,the clearance time showed significant different both in intervention group(F=88.412,P0.01) and control group(F=357.925,P0.01);there were also significant differences in accelerating clearance rate(F=10.726,P0.001) and effective ratio(x2=8.400,P=0.015) in different parts of the lung both in intervention group and control group.Before and after the test,there were no significant changes both in FEV1/FVC%(81.6 ±9.2)% vs(82.9±8.7)%,(P0.05) and PEF 318.7±56.2ml vs 305.3±69.3ml,(P0.05).Conclusion The application of the flutter mucus-clearance device shows efficacy and safety in airway mucus clearance.
