摘  要:目的分析少动鞘氨醇单胞菌致颅内感染的临床和细菌学特征,检测少动鞘氨醇单胞菌的药物敏感谱及探讨其可能的氨基糖苷类耐药机制。方法细菌生理生化分析鉴定,药物敏感性试验,聚合酶链反应(polymerasechain reaction,PCR)扩增方法检测10种氨基糖苷类耐药基因。结果少动鞘氨醇单胞菌对氨基糖苷类抗菌药物和多种头孢菌素类抗菌药物耐药。PCR扩增常见氨基糖苷类耐药基因未获阳性结果。结论少动鞘氨醇单胞菌对多种抗菌药物有较高的耐药性,对氨基糖苷类耐药尚属少见,需要进一步深入研究其耐药机制,与此同时临床上需要关注条件致病菌耐药性的进化。Objective To analyze the clinical and bacterial characteristics of intracranial infection caused by Sphingomonas paucimobilis and understand the antimicrobial susceptibility pattern of S. paucimobilis and its possible mechanism of aminoglycoside resistance. Methods The identification and drug susceptibility test of the isolates were performed by using BD Phoenix 100 species identification system and 10 aminoglycoside-resistance genes were detected by using PCR amplification. Results The identification results indicated that all the pathogens isolated from cerebrospinal fluid samples of a patient on three occasions were S. paucimobilis, which were resistant to aminoglycosides and most cephalosporins. PCR amplification results of the 8 aminoglycoside-resistance genes from the strains were all negative. Conclusion S. paucimobilis has acquired resistance to aminoglycosides, and the resistance mechanism should be clarified. Meanwhile, more attention should be paid to the resistance evolution of conditional pathogenic bacteria.
