硬膜外镇痛泵联合控释性药物对分娩的影响

摘　　要：目的探讨硬膜外镇痛泵(PCEA)联合控释性药物在足月妊娠引产中的临床效应。方法随机将400例单活胎、具备引产指征的初产妇分为四组,分别施予缩宫素、普贝生、缩宫素联合PCEA和普贝生联合PCEA 4种处理方式。结果普贝生联合PCEA组在宫颈成熟度、用药至临产时间、用药至破膜时间、用药至分娩时间、总产程、12(24)h分娩率、阴道分娩率、剖宫产率、住院时间、住院费用以及疼痛等方面与缩宫素组、普贝生组和缩宫素联合PCEA组比较,差异均有统计学意义(P<0.05);降低疼痛和缩短总产程时间是普贝生联合PCEA组的主要优势,其与缩宫素组、普贝生组、缩宫素联合PCEA组比较,差异有统计学意义(P<0.01)。结论普贝生联合PCEA在促进宫颈成熟、提高自然分娩率、减少分娩痛苦等方面具有明显优势,且对母婴无明显不良影响,值得在临床上推荐。Objective To investigate the clinical effect of patient controlled epidural analgesia（PCEA）combined with controlled release drugs in the induced labor of term pregnancy.Methods 400 primiparas of single living fetus with induced labor indications were randomly divided into four groups and separately given 4 processing modes of oxytotin,dinoprostone,oxytotin combined PCEA and dinoprostone combined PCEA.Results The cervical ripening,time from medication to labor,time of medication to onset of delivery,time of medication to rupture of membrane,time of medication to delivery,total stage of labor,delivery rate of 12（24） h,vaginal delivery rate,cesarean section rate,length of hospital stay,hospitalization costs and pain in the dinoprostone combined PCEA group had statistical differences compared with the oxytotin group,the dinoprostone group and the oxytotin combined PCEA group（P0.05）.Reducing pain and shortening the total stage of labor were the main superiority in the dinoprostone combined PCEA group,which showed statistical difference compared with the oxytotin group,the dinoprostone group and the oxytotin combined PCEA group（P0.01）.Conclusion Dinoprostone combined PCEA has significant superiority in promoting cervical ripening,increasing natural delivery rate and reducing delivery pain without obvious adverse influence on mother and newborn,which deserves to be promoted clinically.
