单胎头位脐带绕颈经阴道分娩的可行性研究

摘　　要：目的探讨单胎头位脐带绕颈经阴道分娩的可行性。方法回顾性分析2010年1月至2011年12月经阴道试产的324例脐带绕颈病例(研究组)及180例选择性剖宫产孕妇(对照组)的临床资料。结果研究组胎儿窘迫率及新生儿窒息率与对照组比较,差异无统计学意义(P>0.05)。随脐带绕颈周数增多,胎儿窘迫率及新生儿窒息率增高,阴道助产率及急诊剖宫产率亦增高。结论对于无产科并发症的单胎头位脐带绕颈孕妇,在严密的产程监护下行阴道试产是可行的。但脐带绕颈3周或3周以上应放宽剖宫产指征,以防止胎儿发生意外。Purpose To study the feasibility of vaginal birth in single fetus of cephalic presentation with umbilical cord around neck.Methods The clinical data in 324 cases of umbilical cord around neck by vaginal trial production（study group） and 180 cases of elective caesarean section（control group） from January 2010 to December 2011 were analyzed retrospectively.Results There was no statistical difference in fetal distress rate and the neonatal asphyxia rate between the two groups（P0.05）.Vinginal trial production under close monitoring was feasible to primipara with single fetus of cephalic presentaion and without obstetrical complications.Conclusion With the increase of cycles of umbilical cord around neck,the rates of fetal distress and neonatal asphyxia are increased,the rates of emergency cesarean section and assisted vaginal delivery also are increased,so the indication for cesarean section should be relaxed in primipara with 3 or more than 3 cycles of umbilical cord around neck in order to prevent fetal accident.
