非诺贝特和辛伐他汀治疗中老年高脂血症的临床疗效与安全性分析

摘　　要：目的评价非诺贝特缓释胶囊和辛伐他汀片治疗中老年混合型高脂血症的疗效和安全性,为临床提供用药参考。方法收集76例中老年高脂血症患者,随机分成两组:非诺贝特组和辛伐他汀组,各38例,连续治疗2个月。根据患者的实验室检查结果,判断非诺贝特和辛伐他汀的疗效,记录两组的不良反应,并采用统计学方法进行比较。结果非诺贝特组和辛伐他汀组总有效率分别为86.84%(33/38)和89.47%(34/38),不良反应发生率分别为26.32%和23.68%,两组疗效比较,差异无统计学意义(P>0.05)。辛伐他汀降低总胆固醇(TC)和低密度脂蛋白胆固醇(LDL-C)的效果优于非诺贝特(P<0.05),非诺贝特降低三酰甘油(TG)和升高高密度脂蛋白胆固醇(HDL-C)的效果优于辛伐他汀(P<0.05)。非诺贝特组和辛伐他汀组的治疗成本分别为171.4元和117.6元(P<0.05)。结论与辛伐他汀片相比,非诺贝特缓释胶囊治疗中老年混合型高脂血症的效果与其相当,但是治疗成本较高。对于以TC升高为主的高脂血症的治疗推荐首选辛伐他汀,以TG升高为主的高脂血症的治疗首选非诺贝特。Objective To investigate the effectiveness and safety of fenofibrate and simvastatin in the treatment of middle-aged and elderly mixed hyperlipidemia to provide reference for clinical medication.Methods 76 middle-age and elderly patients with mixed hyperlipidemia were randomly divided into 2 groups treated with fenofibrate and simvastatin,respectively.According to the symptom and the results of examination,the clinical efficacy of two drugs were judged.The adverse reactions were recorded and compared between the two groups by the statistical method.Results The total effective rates of fenofibrate and simvastatin were 86.84% and 89.47%,respectively and the adverse reaction rates were 26.32% and 23.68% respectively.There was no statistically significant difference in the curative effect（P0.05）.TC and LDL-C declined more significantly in the simvastatin group than in the fenofibrate group,and the effect of TG induction in the fenofibrate group was better than that in the simvastatin group and HDL-C rose more significantly in the fenofibrate group than in the simvastatin group（P0.05）.The treatment costs of the fenofibrate group and the simvastatin group were 171.4 Yuan and 117.6 Yuan,respectively.Conclusion Fenofibrate and simvastatin are both effective in the treatment of middle-aged and elderly mixed hyperlipidemia,but Simvastatin Sustained Release Capsule has higher treatment cost.Simvastatin Tablet is suitable for the patients with hyperlipidemia dominated by TC increase,while Tenobrate Tablet is suitable for the patients with hyperlipidemia dominated by TG increase.
