腹泻型肠易激综合征患者中医体质类型与相关影响因素的研究

摘　　要：目的:探讨腹泻型肠易激综合征患者中医体质类型与相关影响因素。方法:采用标准化的9种中医体质分类量表对50例肠易激综合征腹泻型(IBS-D)患者进行中医体质辨识,并就体质类型与性别、诱发因素、年龄、病程、文化水平以及职业分布的相关性进行分析。结果:体质类型最常见为阳虚质、气郁质,其次为痰湿质、湿热质;男性患者湿热质明显高于女性,女性气郁质明显高于男性,差异有统计学意义(P=0.05);诱发因素以饮食不节最常见,多数以进食生冷水果及饮品诱发;其次为受凉因素;阳虚质患者多因进食生冷、受凉而诱发,女性患者月经来潮亦为常见诱发因素;不同中医体质类型与年龄、病程、文化水平以及职业分布无明显相关性。结论:IBS-D与中医体质关系密切,主要分布在阳虚质、气郁质、痰湿质及湿热质4种体质类型中,余下5种体质类型分布较少;中医体质类型的相关因素是性别和诱发因素,而与年龄、病程、文化水平及职业分布无关。Objective:To explore the correlation between TCM Constitution classification of IBS-D patients and related factors. Methods:Standardized classification measurement questionnaire of nine constitutions in Chinese medicine was used to recognize the Chinese medicine constitution of 50 IBS-D patients,then analyze the relationship between constitutions with sex, precipitating factors, age, course of disease, cultural level and profession. Results:The most common type of constitutions were yang-deficiency type and qi-depression type, the second were phlegm-damp type and wet-heat type . Male in wet-heat type were more than female, female in qi-depression type were more than male; The precipitating factors were mostly occupied by improper diet, most of which were having cold fruit or drinks; then were cold weather. Patients of yang-deficiency type were induced mostly by catching cold or having cold, and usually menses were also for precipitating factors in female; There was no significant correlation between age, course, cultural level ,profession and constitution of Patients. Conclusion:IBS-D have a close relationship with TCM constitution. IBS-D patients mainly distributed in four constitutions, they are yang-deficiency type, qi-depression type, phlegm-damp type and wet-heat type,the other five kinds of constitutions were less distributed. The TCM constitution classification are related with the factors such as sex and precipitating factors, but no relationship with age, course of disease, cultural level and profession.
