动力髋螺钉系统治疗老年股骨转子间骨折的临床疗效观察

摘　　要：目的探讨动力髋螺钉(dynamic hip screw,DHS)系统治疗老年转子间骨折的手术策略,并对相关影响因素进行分析。方法 1997年1月—2011年6月,采用DHS对98例60岁以上的老年转子间骨折患者进行治疗,除7例在随访期内因内科合并症死亡外,其他均获得随访,平均随访时间31(12～48)个月。按照黄公怡临床评定标准进行评估。结果平均骨折愈合时间为24(22～28)周;根据黄公怡临床评定标准优64例、良17例、可10例,优良率89.01%(81/91)。结论 DHS系统具有固定效果好、手术创伤小等优点,对于老年转子间骨折患者是一种疗效非常肯定的内固定系统。但面对此类患者,应从患者的机体功能、内科合并症、骨折的分型等诸多方面进行综合考虑,以选择最合理的治疗方式。Objective To explore the surgical strategy of using dynamic hip screw （DHS） system to treat aged patients with the intertrochanteric fracture, and the related factors were analyzed. Methods From January 1997 to June 2011,98 intertrochanteric fracture cases were treated with DHS and followedup,the mean follow-up time was 31 （12-48） months. During the follow-up time, 7 cases died of the medical complications. All cases were evaluated by the clinical evaluation standard of HUANG Gong-yi. Results The mean cure-time of fracture was 24 （22-28） weeks, the number of excellent cases was 64, the good was 17 and the medium was 10. The good-excellent value was 89.01% （81/91）. Conclusion DHS system could fix aged patients with the intertro- chanteric fracture effectively and decreased the surgical trauma to a relative low level. For the aged patients with intertrochanteric fracture, DHS system was a worthful interfixation instrument. But to choose the best effective treatment strategy for the aged patients with intertrochanteric fracture, other factors, such as the general condition, the medical complications, the type of fracture and so on, should be considered.
