关节镜下双束重建与单束重建膝前交叉韧带的临床对比分析

摘　　要：目的探讨关节镜下双束重建与单束重建膝前交叉韧带的临床疗效。方法选取本院2010年1月～2011年8月收治的膝前交叉韧带损伤患者81例,根据手术方法将其分为两组,38例患者采用关节镜下单束重建为对照组,43例患者采用关节镜下双束重建为观察组,分别于治疗前和治疗12个月后给予患者IKDC膝关节主观评分、Lysholm评分、Tegner体育量表评分。结果治疗后,观察组与对照组IKDC膝关节主观评分、Lysholm评分、Tegner体育量表评分均明显升高,其中观察组IKDC膝关节主观评分、Tegner体育量表评分均明显高于对照组,差异均有统计学意义(P<0.05)。观察组Lysholm评分高于对照组,差异无统计学意义(P>0.05)。结论关节镜下双束重建与单束重建膝前交叉韧带均有显著的临床疗效,其中双束重建能更好地满足膝关节的生物力学要求,具有更高的稳定性,值得临床推广使用。Objective To investigate the clinical curative effect on double bundle reconstruction and single bundle reconstruction under arthroscopic in the treatment for anterior cruciate ligament of knee. Methods Eight-one patients with knee anterior cruciate ligament injury were selected in our hospital from January 2010 to August 2011. According to the operation method, patients were divided into two groups. 38 patients used single bundle reconstruction under arthroscopic as the control group. 43 patients used double bundle reconstruction under arthroscopic as the observation group. IKDC score, Lysholm score, Tegner score were compared between two groups before treatment and after 12 months treatment. Results After treatment, IKDC score, Lysholm score, Tegner score in the observation group and control group increased significantly. IKDC score, Tegner score in the observation group were significantly higher than the control group, the difference was statistically significant （P 〈 0.05 ）. Lysholm score in the observation group was higher than the control group, the difference was not statistically significant （P 〉 0.05）. Conclusion Double bundle reconstruction and single bundle reconstruction under arthroscopic in the treatment for anterior cruciate ligament of knee have significant clinical curative effect. Double bundle reconstruction can better satisfy knee joint biomechanics requirements, which has higher stability. That is worthy of clinical use.
