摘  要:目的观察慢性阻塞性肺疾病(COPD)合并肺动脉高压(PH)和非PH患者血中C反应蛋白(CRP)和中性粒细胞百分比(%)的水平,探讨炎症反应在COPD相关PH发病机制中的意义。方法将60例COPD急性加重期患者,根据肺动脉收缩压(PASP)≥30 mmHg,分为COPD合并PH组和COPD非PH组各30例,同期健康体检者30例,比较3组间血CRP和N%水平的差异。结果 COPD合并PH组和COPD非PH组血CRP、N%水平均较健康对照组明显升高(P<0.05);血清CRP水平在PH组和非PH组间差异有统计学意义(P<0.05),而血N%水平在上述2组间差异无统计学意义(P>0.05)。PH组血清CRP水平与PASP呈正相关(r=0.377,P<0.05)。结论 CRP、N%均是反映COPD炎症反应的重要指标,其中CRP更敏感,且在PH的形成过程中起着十分重要的作用。Objective To investigate the blood C - reactive protein (CRP) and neutrophil percentage (%) levels in COPD patients with and without pulmonary hypertension (PH), and to explore the significance of the inflammatory response in COPD - related PH. Methods 60 patients with COPD at acute exacerbation were divided into COPD with PH group and COPD without PH group according to the pulmonary artery systolic pressure (PASP) ≥30 mmHg. 30 healthy subjects were selected as control group. The blood CRP and N% levels were compared in three groups. Results The blood CRP and N% levels in COPD with PH group and COPD without PH group were significantly higher than those in control group ( P 〈 0.01 ) ; the serum CRP level in the PH group was significantly higher than those in COPD without PH group (P 〈0.01 ), and there was no significant differences in blood N% level between two groups. The serum CRP level of the PH group was positively correlated with PASP ( r = 0. 377,P 〈 0.05). Conclusion CRP and N% are important indicators to reflect the inflammatory response of COPD, CRP is more sensitive and plays an important role in the PH.
