抗核糖体P蛋白抗体与系统性红斑狼疮狼疮性肾炎的临床研究

摘　　要：目的:探讨抗核糖体P蛋白抗体和系统性红斑狼疮(SLE)狼疮性肾炎(LN)、抗核糖体P蛋白抗体与LN病理分型的关系。方法:收集行肾活检的60例SLE患者临床资料,分析抗核糖体P蛋白抗体与LN的关系及与LN病理分型的关系。结果:(1)抗核糖体P蛋白抗体与LN(临床指标)存在关系(OR=3.48,P<0.05),抗核糖体P蛋白抗体与抗Ds-DNA抗体协同时也与LN相关(P<0.05)。(2)抗核糖体P蛋白抗体阳性组与抗核糖体P蛋白抗体阴性组相比较,Ⅴ型LN在抗核糖体P蛋白抗体阳性组有更高的发生频率(分别为75.0%和32.5%),差别具有统计学意义(χ2=4.25,P<0.05)。结论:抗核糖体P蛋白抗体和活动性LN可能具有一定的联系,且抗核糖体P蛋白抗体与Ⅴ型LN也存在一定联系,此结论对临床医生判断是否有LN活动及可能的LN病理类型具有一定的指导意义。Objective： To study the associations between antibodies to ribosomal P proteins and lupus nephritis（ LN ）clinically, especially the associations between antibodies to ribosomal P proteins and histopathologic patterns of lupus nephritis. Methods： Sixty patiets of systemie lupus erythematosus （SLE） with biopsy of kidney were selected, At the same time, the materials associated with patients were collected and the associations between antibodies to ribosomal P proteins and lupus nephritis, especially the associations belween antibodies to ribosomal P proteins and pathology of lupus nephritis were analyzed. Results： Firstly： antibodies to ribosomal P proteins were associated with lupus nephritis （clinically） （0R：3,48, P〈0.05 ）. Secondly： Compared patients of SLE with anti-ribosomal P proteins anti-bodies amt those without anti-ribosomal P proteins antibodies ,the patients with anti-ribosomal bodies had a higher frequency of class V lupus nephritis（75.0% vs 32.5%, Chi Square：4.25, P〈0.05）. Conclusion： There may be associations between antibodies to ribosomal P proteins and active lupus nephritis, especially the associations between antibodies to ribosomal P proteins and class V lupus nephritis ,which may contribute to recognizing the active lupus nephritis and class V lupus nephritis clinically.
