摘  要:目的观察氨茶碱对幼兔胎粪吸入综合征临床参数的影响。方法 18只日龄20~30 d的健康日本大耳幼兔随机分为三组,空白组(n=6),对照组(n=6),氨茶碱组(n=6)。空白组气管插管后不灌胎粪;后两组气管插管后注入4 mL/kg的胎粪混合物45 mg/mL,氨茶碱组在胎粪吸入综合征模型制备成功后0.5 h和2.5 h静脉注射氨茶碱(2.0 mg/kg),对照组在同样时间静脉注射等量的生理盐水。三组均予同步间歇指令通气(SIMV)模式通气支持,以维持正常潮气量范围(6~8 mL/kg)、正常血氧饱和度(85%以上)及正常血气值为准。通气5 h后记录呼吸频率(RR)、心率(HR)、平均气道压(MAP)、动态肺顺应性(Cydn)和血气分析值,计算氧合指数(OI)和动脉/肺泡氧分压比值(a/APO2)。结果①3组间PH值(F=0.651)和动脉血二氧化碳分压(PCO2)值(F=0.651)比较差异均无统计学意义(均P>0.05),氨茶碱组和对照组比较差异均无统计学意义(均P>0.05)。②三组间动脉血氧分压(PO2)值(F=301.460)、OI值(F=110.300)和a/APO2比值(F=197.600)比较差异均有统计学意义(均P<0.05),氨茶碱组和对照组比较差异均有统计学意义(均P<0.05)。③氨茶碱组和对照组间RR(Z=0.260)和HR(Z=0.211)比较,差异均无统计学意义(均P>0.05)。④氨茶碱组和对照组间MAP值(Z=6.826)和Cydn值(Z=-3.101)比较,差异均有统计学意义(均P<0.05)。结论氨茶碱能改善兔胎粪吸入时的肺部氧合状态,可降低气道平均压和增加肺整体顺应性。Objective To observe the effect of Aminophylline on clinical parameter of young rabbits with severe meco- nium aspiration syndrome. Methods 18 cases of healthy Japanese young rabbits aged 20-30 days were randomly di- vided into three groups: blank group(n = 6), control group(n = 6), aminophylline group(n = 6). The blank group were did not inject meeonium into their lung after trachea eannula, and the rabbits of control group and the aminophylline group were both injected meconium of 4 mL/mg (45 mg/mL) by endotraeheal intubation. The rabbits of aminophylline group were injected with aminophlline (2.0 mg/kg) into vein at 0.5 hour and 2.5 hours after the model of MAS estab- lished. And the control group was injected with the same capacity of the physiological saline at the same time. The normal tidal volume (6-8 mL/kg), the normal blood oxygen saturation (〉85%) and the normal blood gas value of rab- bits of three groups were maintained by synchronized intermittent mandatory ventilation. The respiration rates (RR), the heart rates (HR), the mean arterial pressureon (MAP), the dynamic lung compliance (Cydn) and the blood gas volue of rabbits of three groups were recorded after ventilation for 5 hours, then the oxygenation index (OI) and the arterial oxygen pressure/alveolar oxygen partial pressure ratio (a/APQ) could be caculated. Results (1)There was no signifi- cantly different in the PH (F = 0.651) and the PaC02 (F = 0.651) of three groups, there was no significantly different between aminopylline group and control group (all P 〉 0.05). (2)The Pa02 (F=301.460), the OI(F = 110.300) and the a/ APO2 (F = 197.600) of three groups was significantly different, there was significantly different between the aminopy- lline and the control group (all P 〈 0.05).
