摘　　要：目的探讨真空辅助微创活检加置管冲洗引流治疗慢性乳腺炎的操作技巧和临床优势。方法 2010年6月～2012年5月,超声引导下应用真空辅助微创活检系统加旋切刀对肿块型与脓肿型慢性乳腺炎局部病灶进行切除,残腔冲洗及置管引流,观察其治愈率、治愈时间和复发率。结果 32例一期治愈率96.9%(31/32),仅1例在手术后5天换药仍可见脓液溢出,为术后引流不畅,扩大引流口后22天痊愈。随访时间2～24个月,中位数10个月,局部复发率3.1%(1/32)。结论应用真空辅助微创活检系统可在超声引导下对慢性乳腺炎局部病灶进行大范围甚至完全切除,辅以置管冲洗引流,可很快获得良好的治疗效果。Objective To study the technique and advantages of vaccum-assisted breast biopsy combined with douching and drainage for the treatment of chronic mastitis. Methods From June 2010 to May 2012, under ultrasound guidance, we removed breast lump or abscess in 32 patients with chronic mastitis by using a vaccum-assisted biopsy system plus a rotating surgical cutter, douching and drainage were performed after the surgery. The cure rate, cure time and recurrence rate were observed. Results The one-session cure rate was 96.9% （31/32）. Only one patient still had purulent secretion in 5 days after the operation, which was caused by inadequate drainage. We therefore extended the drainage incision, and then the patient was cured in 22 days. The patients were followed for 2 to 24 months with a medium of 10 months. The rate of local recurrence was 3. 1% （1/32）. Conclusions Under ultrasound guidance, vaccum-assisted biopsy system can totally or subtotally remove the lump or abscess of chronic mastitis. With douching and drainage, the patients are cured quickly.
