妊娠早中期部分血常规指标变化与妊娠糖尿病发生的相关性分析

摘　　要：目的:探讨妊娠早中期血常规白细胞(WBC)总数、血小板(PLT)计数、红细胞(RBC)计数及血红蛋白(HGB)含量变化与妊娠糖尿病(GDM)发生的相关性。方法:分别在孕妇首次孕检即孕12～14周及孕24～28周行糖筛查时,予血常规检查,测定WBC、PLT、RBC及HGB。将葡萄糖糖耐量检查确诊为GDM的78例孕妇作为病例组(GDM组),糖耐量正常的683名孕妇作为对照组(Con组),分别比较两组早、中期WBC、PLT、RBC等计数及HGB水平与GDM发生的相关性进行统计学分析。结果:GDM组早期外周血白细胞计数、红细胞计数、血红蛋白明显高于对照组(P<0.05),血小板计数无明显差异。GDM组中期外周血红细胞计数、血红蛋白含量明显高于对照组(P<0.05),中期两组外周血白细胞、血小板计数均升高,无统计学差异。GDM组早中期血小板计数明显下降,患者高危并发症明显增加。结论:孕早期外周血白细胞计数、红细胞计数、血红蛋白含量明显升高与GDM患病正相关(P<0.05);孕中期外周血红细胞计数和或血红蛋白含量升高与GDM患病正相关(P<0.05);GDM患者血小板计数下降是疾病的独立危险因素。Objective To investigate the relation between changes in white blood cell （WBC） count, platelet （PLT） count, red blood cell（RgC） count and the level of hemoglobin （HGB） in early and middle pregnancy and gestational dia- betes mellitus（GDM）. Methods WBC,PLT,RBC and H GB were tested when blood glucose screenings were performed at first pregnancy test, that is, pregnancy 12-14 weeks and pregnancy 24-28 weeks respectively. 78 cases of pregnant women who were diagnosed GDM through glucose tolerance test were enrolled as disease group（GDM group）,683 cases pregnant women with normal glucose tolerance control group（Con group）. The relation of the two groups in early and middle pregnancy between WBC,PLT,RBC counts and the level of HGB and GDM was compared respectively,and statistical analyses were performed. Results The WBC count, RBC count and HGB in early pregnancy in GDM group were significantly higher than those in control group （P^0.05）, yet the difference in PLT count was not significant. The RBC count,the level of HGB in middle pregnancy in GDM group were significantly higher than those in control group（P〈0.05）. The WBC,PLT counts of the two groups increased,while the differenee was not significant. The high risk complications of patients in GDM group in early and middle pregnancy whose PLT count decreased signifi- cantly increased significantly. Conclusfons The significant increases of WBC count, RBC count, level of HGB in early pregnancy are positively correlated with GDM（P〈0.05） ;the increases of RBC count and the level of HGB in middle pregnancy are positively correlated with GDM （P〈0.05） ;the decrease of PLT count in patients of GDM is independent risk factor of disease.
