摘　　要：目的:确定BED捕获酶联免疫试验(BED法)检测发现HIV新近感染者的灵敏度和特异度。方法:以云南省德宏州感染时间>168 d的HIV感染者为"真正的"既往感染者,以感染时间≤168 d的HIV感染者为"真正的"新近感染者,对其血清标本采用BED法进行检测。结果:29例新近感染者中BED检测阳性共24例,灵敏度为82.76%;264例既往感染者中BED检测阴性共218例,特异度为82.58%;感染者感染时间越长,BED特异度越高,而对于感染时间在169 d～365 d的感染者,BED特异度低、假阳性率高。结论:BED法在确定HIV新近感染者时具有较高的灵敏度和特异度,但对部分感染者存在一定的误判率。Objective：To determine the sensitivity and specificity of BED-capture enzyme immunoassay（BED） in identifying recent HIV infections.Methods： HIV/AIDS cases infected at least 169 days ago were treated as ＂true＂ old or former HIV infectors,while HIV cases who were infected with HIV within 168 days were regarded as ＂true＂ recent HIV infectors.Serum samples of these confirmed ＂true＂ old and recent HIV infectors were tested by BED.Results： Twenty-four cases of twenty-nine true recent HIV infectors were positive by the BED test,the sensitivity of BED was 82.76%.Two hundred and sixty-four cases of 264 ＂true＂ old HIV infectors were negative for the BED test,the specificity of BED was 82.58%.The specificity of BED-CEIA was increasing with the HIV infection time of participants.The specificity was very low while the false positivity was very high among old HIV infectors whose infection time was within 169-365 days.Conclusion： The overall sensitivity and specificity of BED were relatively high in identifying recent HIV infections.Cautions should be given to a substantial proportion of false positives or false negatives of BED in identifying recent HIV infections among certain subgroups of HIV-infected patients.
