前列腺结核MRI诊断的临床价值

摘　　要：目的探讨前列腺结核的MRI表现特点。方法回顾性分析经手术病理证实的前列腺结核病例6例。5例有肺结核病史。临床症状主要是尿频、排尿不畅、血尿,PSA水平升高不明显。结果本组前列腺结核病灶呈不规则形,边界不清,T1WI呈等信号影,T2WI呈稍高信号影,DWI呈稍高信号,ADC值约为(1.3～1.5)×10-3 mm2/s,增强后不均匀强化。前列腺结核常形成脓肿,脓肿区T1WI呈稍低信号,T2WI呈稍高信号,DWI呈明显高信号影,ADC值约为(1.0～1.2)×10-3 mm2/s,增强后脓肿壁呈环状强化。前列腺结核易累及邻近组织,一般无盆腔淋巴结肿大以及骨骼破坏。结论前列腺结核MRI表现有一定特征性,密切结合临床病史可提高诊断准确性。Objective To observe imaging feature of the prostatic tuberculosis.Methods 6 patients with prostatic tuberculosis proven pathologically were retrospectively analyzed.Their clinical symptoms included frequent micturition,urination unfreely,and blood urine.And the level of PSA was usually normal.Results The features of prostatic tuberculosis in MR included irregular shape,unclear border,equal signal in T1WI,slightly high signal in T2WI and DWI,and obviously enhancement.The mean ADCs for the tuberculosis focus were about（1.3~1.5）×10-3mm2/s.The abscesses often taken form,and their features were slightly low signa in T1WI,high signal in T2WI and DWI,and cyclic annular＇s enhancement.The mean ADCs for the abscess were（1.0~1.2）×10-3mm2/s.Prostatic tuberculosis often infringed upon the vicinity organizes.Swelling lymph nodes can not be found in the pelvic cavity,and skeletons were destroyed seldomly.Conclusion MR can show some characteristic features of prostatic tuberculosis.
