广西贵港市低档暗娼的高危行为及HIV/STD感染状况调查

摘　　要：目的调查了解广西贵港市不同场所,低档暗娼的高危行为特征及艾滋病病毒(HIV)和其他性传播疾病(STD)的感染状况。方法于2011年9-10月,对贵港市城乡结合部和辖区乡镇所有娱乐场所的低档暗娼开展一次横断面调查。结果在调查的444人中,洗脚屋/发廊暗娼(A类)占34.23%、旅社/招待所暗娼(B类)占37.39%、小饭店/路边店暗娼(C类)占16.44%、出租屋/赶圩女暗娼(D类)占11.94%;A、B、C和D四类低档暗娼最近1个工作日的嫖客中位数为3个、2个、3个和5个,最近1次与嫖客使用安全套比例依次为80.26%、46.99%、53.42%、62.26%;A、B、C和D四类低档暗娼中HIV/STD也呈依次上升趋势,HIV感染率分别是0.66%、1.81%、4.11%、16.98%,梅毒感染率分别为13.16%、15.66%、17.81%和18.87%。结论贵港市低档暗娼,尤其是更低档的暗娼,嫖客数多,安全套使用率较低,HIV/STD感染率较高,是造成当地艾滋病呈弥漫性扩散的主要原因,低档暗娼是当地艾滋病防控、监测和干预的重点人群。Objective To learn high risk behaviors, the prevanlence of HIV and other STDs among low-grade female sex workers （FSWs） from different venues of Guigang City, Guangxi Province. Methods Cross-sectional survey was carried out among FSWs （N=444） from all venues of the urban and rural connection areas in Guigang City from Septermber to October 2011. Results The proportion of FSWs recruited from the feet-washing house and the salon （type A） was 34.23% and 37.39% in hostel （type B）, 16.44% from small hotels and street hotels （type C）, and 11.4% from rented rooms and the dike woman （type D）. The median number of served male clients was 3, 2, 3 and 5 among FSWs of type A, type B, type C and type D respectively. And their proportion of consistent condom using with male clients in recent working day was 47.37%, 53.61%, 54.79%, and 60.38%, respectively. The prevalence of HIV/STDs all showed an increasing trend among low-grade FSWs of type A, type B, type C and type D, among whom the prevalence of HIV was 0.66%, 1.81%, 4.11%, and 16.98%, respectively. Additional, the prevalence of syphilis was 13.16%, 15.66%, 17.81% and 18.87% , respectively. Conclusions Compared with those of high-grade FSWs, the middle/low grade had more number of male clients in Guigang City, lower condoms using rate, high prevalence of HIV/STDs among them, which was the main reason for the diffuse proliferation of AIDS in locality. So, HIV/STDs surveillance and behaviors intervention should be mainly focused on local low-grade FSWs to prevent further transmission of HIV epidemic to the general population.
