摘  要:目的评价以传染源控制为主的血吸虫病防治对策在高原峡谷地区的防治效果。方法通过在西甸和新庄两个行政村实施健康教育、查治病和化疗、粪水管理、禁牧、查灭螺等以传染源控制为主的血吸虫病防治措施,观察和考核防治效果。结果西甸村血吸虫病人群感染率和家畜感染率分别由2006年的6.43%和1.68%下降到2011年的0.00%,2008年起没有发现新感染病人和14岁以下病人;2011年的钉螺面积、有螺框出现率、查出钉螺只数、活螺平均密度、活螺最高密度等螺情指标分别较2006年下降68.11%、79.44%、84.14%、85.32%、72.22%,2008年以后没有发现阳性钉螺。新庄村血吸虫病人群感染率和家畜感染率分别由2006年的3.45%和0.49%下降到2011年的0.00%,2007年起没有发现新感染病人和14岁以下病人,2008年以来没有发现家畜血吸虫感染;2011年的钉螺面积、有螺框出现率、查出钉螺只数、活螺平均密度、活螺最高密度等螺情指标分别较2006年下降76.60%、19.51%、64.10%、63.80%、74.23%,2007年以后没有发现阳性钉螺。结论以传染源控制为主的血吸虫病综合防治措施在云南鹤庆县高原峡谷型血吸虫病流行区应用效果良好,防治成绩明显。Objective To explore the results of schistosomiasis control strategy focused on the source of infection in plateau and canyon areas in Yunnan province. Methods The main measure in schistosomiasis prevention and treatment including health education, diagnosis and treatment of infections, chemotherapy, feces and water management, grazing forbidden, snails survey and elimination, et al. were carried out in two administrative villages of Xidian and Xinzhuang. Results Schistosomiasis infection rate in population and livestock of Xidian village dropped from 6.43% and 1.68% (2006) to both 0.00% (2011), respectively. Moreover, there was no new infected patient and patients under the age of 14 years; since 2008; the index such as oncomelania distribution areas, percentage of frames with live snail, the number of oncomelania, average concentration of live snails, the highest concentration of live snails in 2011 dropped by 68.11%, 79.44%, 84.14%, 85.32% and 72.22% compared with those in 2006. And there was no positive oncomelania since 2008. Meanwhile, the schistosomiasis infection rate in humans and livestock decreased from 3.45% and 0.49% in 2006 to both 0.00% in 2011, respectively. Moreover, there was no new infected patient and patients under the age of 14 year since 2007 and no infection in domestic animals was detected since 2007. The oncomelania distribution areas, percentage of frames with live snail, number of oneomelania, average concentration of live snails, the highest concentration of live snails in 2011 dropped by 76.60%, 19.51%, 64.10%, 63.80% and 74.23% compared with that in 2006. And no infected oncomelania was detected since 2007. Conclusion The integrated control measures focused on the source of infection is effective in control the prevalence of schistosomiasis in plateau and canyon areas in Yunnan province.
