摘　　要：目的:通过研究髓样细胞触发受体-1(triggering receptor expressed on myeloid cells-1,TREM-1)在铜绿假单胞菌呼吸系统感染和定植不同状态下表达水平的变化,探讨TREM-1与铜绿假单胞菌肺炎及呼吸道定植的相关性。方法:将90只SD大鼠随机分为肺部感染组、口咽部定植组及生理盐水对照组,并分别在3、9、24h运用ELISA法检测血清sTREM-1、C反应蛋白(CRP)和降钙素原(PCT)浓度;病理组织切片观察各组肺部炎症改变。结果:感染组3、9、24h血清sTREM-1、CRP及PCT的水平较定植组和对照组均明显增高(均P<0.05),定植组与对照组各指标比较差异没有统计学意义;感染组血清sTREM-1浓度与CRP、PCT含量呈正相关(相关系数分别为0.86和0.75,均P<0.01);肺组织病理改变在感染组9h最为严重。结论:TREM-1的检测有助于铜绿假单胞菌感染与定植的鉴别;TREM-1对感染性疾病预后判断有指导意义;在感染性疾病的诊断中血清sTREM-1与炎症指标CRP,PCT具有同样重要的意义。Objective To study the expressions of triggering receptor expressed on myeloid cells-1 （TREM- 1 ） in respiratory pseudomonas aeruginosa infection and colonization in different situation, and to investigate the relationship of TREM-1 with pseudomonas aeruginosa pneumonia and colonization. Methods Ninety Sprague-Dawley （SD） rats were randomly divided into three groups ： lung infection group, oropharynx colonization group and normal saline control group. The serum concentrations of sTREM-1, C-reactive protein （CRP） and procalcitonin （PCT） were measured by enzyme-linked immunosorbent assay （ELISA） in 3 h, 9 h,24 h. respectively Lung inflammation pathological changes were observed in tissue slices. Results In each time point, the serum levels of sTREM-1, CRP and PCT in infection group were significantly higher than colonization group and control group （P 〈 0.05）. There were no significant differences between colonization group and control group . There was positive correlation between sTREM-1 with PCT and CRP （Correlation coefficients were 0.86 and 0.75 respectively, P 〈 0.01 ）.The lung tissue pathology changes were the most serious in the infection group at 9 h. Conclusions TREM-1 was contribute to identify the infection and colonization induced by pseudomonas aeruginosa and might help to evaluate the prognosis of infectious diseases. The sTREM-1 had the same important significance as CRP and PCT in diagnosis of infectious diseases.
