肺结核患者血清干扰素-γ、白细胞介素-18和骨桥蛋白水平的动态变化及临床意义

摘　　要：目的:通过检测肺结核患者抗结核治疗前后血清干扰素-γ(IFN-γ)、白细胞介素-18(IL-18)和骨桥蛋白(OPN)水平的动态变化,以探讨其临床意义。方法:选择初治的活动性肺结核患者42例,正常对照组36例。应用双抗体夹心酶联免疫吸附法(ELISA)测定抗结核治疗前、治疗后1个月、治疗后2个月血清IFN-γ、IL-18和OPN的水平。结果:抗结核治疗前患者血清IFN-γ、IL-18和OPN水平明显高于正常对照组;治疗后1个月血清IL-18和OPN水平即开始降低,治疗后2个月血清IL-18和OPN水平均显著降低。但血清IFN-γ水平在抗结核治疗前后无显著变化。结论:动态监测肺结核患者血清IL-18和OPN水平,可作为评估肺结核病变活动性和抗结核药物治疗效果的早期检测指标。Objective To investigate the dynamic changes of interferon-gamma （IFN-gamma）, interleukin -18 （IL-18） and osteopontin （OPN） in serum of patients with pulmonary tuberculosis before and after anti-tuberculosis treatment and its clinical significance. Methods Totally 42 patients with untreated pulmonary tuberculosis in our hospital and 36 normal controls were selected. Enzyme-linked immunosorbent assay （ELISA） was used to assay the levels of IFN-γ, IL-18 and OPN in the selaxm samples before and after anti-tuberculosis treatment for one or two months. Results The serum levels of IFN-γ,IL-18 and OPN in patients with active puhnonary tuberculosis were higher obviously than those in control group. The serum levels of IL-18 and OPN began to decrease after treatment for 1 month, and decreased significantly after treatment for 2 months. However, the serum levels of IFN-γ were not changed significantly before and after anti-tuberculosis treatment. Conclusion Dynamic monitoring the serum levels of IL-18 and OPN in patients with active pulmonary tuberculosis can be used as early indicators for assessment of tuberculosis disease activity and therapeutic effects of anti-TB drugs.
