摘 要:目的:通过股骨髁形态和股骨曲度的测量分析,探讨股骨远端形态与动力髁螺钉的适配性。方法:男女性股骨标本各30根,测量内侧髁、外侧髁、髁间窝高度及股骨髁干角。X线摄片30位正常成年志愿者股骨,进行冠状面上角度测量。结果:男性股骨内侧髁高、外侧髁高、髁间窝高各值均大于女性,股骨髁干角女性大于男性(P<0.01~P<0.05)。股骨干与股骨远侧段之间的夹角、股骨干与股骨近侧段之间的夹角存在显著的性别差异(P<0.01)。拟合股骨标本和动力髁螺钉,两者男性存在轻度不合适4例,明显不合适2例,女性分别为7例和2例。结论:动力髁螺钉设计应有性别差异,术中应根据股骨的曲度对钢板进行预弯曲。Objective To discuss the suitability between the distal femoral form and dynamic condylar screw by measuring the femoral condyle form and curvature of femur. Methods In 30 male and 30 female femur specimens, the height of internal condyle, lateral condyle and fossa intercondyloidea were measured, and femoral condyle shaft angles were recorded. Then femurs of 30 normal adult volunteers were taken by X-ray to measure the angles of coronal plane. Results The height of internal condyle, lateral condyle and fossa intercondyloidea in male were significantly higher than those in female, while the femoral condyle shaft angles in female was higher (P 〈 0.01 or P 〈 0.05). The angles between femoral shaft and femur distal segment, and between femoral shaft and femur proximal segment were significantly different between male and female (P 〈 0.01 ). After fixing femoral form specimens and DCS, there were 4 mild inappropriate and 2 obviously inappropriate cases in male, 7 and 2 cases in female, respectively. Conclusion There should be gender differences in the design of DCS, and the steel plate should be pre-buckled to suit for curvature of the femur during the operation.
