实施国家基本药物制度对山东省某县乡镇卫生院的影响调研及政策建议

摘　　要：目的:探讨实施国家基本药物制度对乡镇卫生院的影响,为完善相关政策提供建议。方法:通过问卷调查(整群抽样)和访谈,收集山东省G县各乡镇卫生院2009-2011年卫生服务提供和经济运行数据,并进行分析和讨论,进而提出建议。结果:国家基本药物制度实施后,样本乡镇卫生院药品平均价格降低了37.93%,门诊次均费用降低了44.38%,住院次均费用降低了54.87%,年药品收入下降了31.87%,总收入减少了5.37%,药品收入比例由49.75%降到28.54%,同时门诊和住院就诊人次分别减少了3.90%、4.70%。结论:实施国家基本药物制度后乡镇卫生院药品价格降低,诊疗费用降低,收入减少且收入结构改变,同时就诊人次减少。后续应考虑建立长效常态的补偿机制,适当调整医疗服务价格,完善基本药物目录并逐步推进其他配套措施改革。OBJECTIVE： To investigate the impact of National Essential Drug System （NEDS） on township hospitals, and to provide suggestions for perfecting relevant policy. METHODS： Through questionnaire survey （cluster sampling） and key informant interview, the data of health service and economics operation were collected from township hospitals in G county of Shandong prov- ince during 2009-2011, and then discussed and analyzed, Some suggestions were put forward. RESULTS： After the implementa- tion of National Essential Drug System, the average price of drugs reduced by 37.93% ; the medical expenses per outpatient visit re- duced by 44.38% ; the medical expenses per inpatient visit reduced by 54.87% ; annual drug income reduced by 31.87% and total income decreased by 5.37% as well as the percentage of drug income in total revenue reduced from 49.75% to 28.54%. In addi- tion, the number of the outpatients and inpatients reduced by 3.90% and 4.70%, respectively. CONCLUSION： After the implemen- tation of National Essential Drug System, the drug price and medical expenses per patient visit have been reduced. The total hospi- tal income reduces and its structure also changes. Moreover, the number of patients reduces. It is essential to build a long-acting compensation mechanism, adjust the charge of health service and improve essential medicine list and the reform of other support measures.
