罗哌卡因复合舒芬太尼腰硬联合麻醉在重度子痫前期剖宫产术中的应用

摘　　要：目的评价罗哌卡因复合舒芬太尼腰硬联合麻醉在重度子痫前期产妇剖宫产术中的应用。方法 45例重度子痫前期产妇随机分为3组,A组蛛网膜下腔注入0.5%罗哌卡因7.5mg复合舒芬太尼5μg,B组注入0.5%罗哌卡因7.5mg,C组注入0.5%罗哌卡因10.5mg。观察麻醉效果及麻醉不良反应。结果 A、C两组麻醉优良率明显高于B组(P<0.05)。C组麻醉后低血压的发生率明显高于A、B组(P<0.05)。结论罗哌卡因复合舒芬太尼腰硬联合麻醉用于重度子痫前期产妇剖宫产手术,术中麻醉效果好,血流动力学稳定,值得临床推广。Objective To assess the application of combined spinal and epidural anesthesia（CSEA） with ropivacaine combined with sulfentanil in patients with severe preeclampsia.Methods Forty-five patients with severe preeclampsia were randomLy divided into three groups,group A（intrathecal 0.5%Ropivacaine 7.5 mg combined with 5 μg sulfentanil）,group B（0.5%Ropivacaine 7.5 mg）and group C（0.5% Ropivacaine 10.5 mg）.The quality of anesthesia and side-effects were investigated.Results The good rate of the quality of anesthesia in group B was significantly lower than that in group A and C（P0.05）,The incidence of hypotension in group C was higher than that in A and B（P0.05）.Conclusion CSEA with intrathecal ropivacaine combined with sulfentanil is a piece of safe and effective technique suitable for cesarean delivery in severely preeclamptic patients,with the stable haemodynamics.
