曲美他嗪在老年冠心病慢性心力衰竭中的临床应用

摘　　要：目的探讨曲美他嗪治疗老年冠心病伴慢性心力衰竭(CHF)的临床疗效及安全性。方法选择我院住院的老年冠心病伴CHF患者60例患者,随机分为治疗组30例和对照组30例。对照组采用常规药物治疗;治疗组在常规治疗基础上加服曲美他嗪,治疗6个月观察2组治疗前后临床疗效、左室舒张末内径(LVEDD)、左室射血分数(LVEF)、6 min步行试验及药物不良反应等。结果治疗6个月后,治疗组的总有效率83.3%,对照组为63.3%,两组比较差异有统计学意义(P<0.05);在LVEDD、LVEF、6 min步行距离改善的程度等方面,治疗组均优于对照组(P<0.05);曲美他嗪无明显的治疗不良反应。结论在常规治疗的基础上加用曲美他嗪治疗老年冠心病慢性心力衰竭效果显著,能提高患者的心功能,且无严重不良反应。Objective To explore the clinical effect of trimetazidine on chronic congestive heart failure due to coronary atherosclerotic heart disease in aged people.Methods Sixty patients with chronic congestive heart failure due to coronary atherosclerotic heart disease in our hospital were randomly divided into control group and treatment group.There were 30 cases of patients in each group.Regular drug therapy was given to patients in control group,while trimetazidine was applied to patients in treatment group besides regular drug therapy.After 6-month therapy course,the left ventricular end-diastolic diameter（LVEDD）,left ventricular ejection fraction（LVEF）,6 min walking test,and adverse drug reactions of the two groups were evaluated pre and post treatment.Results After 6-month treatment,the total effective rate was 83.3%in treatment group and 63.3% in control group.The difference had statistical significance（P0.05）and therapeutic effect was preferable in observation group（P0.05）.As for the cardiac function indexes,including LVEDD,LVEF and 6 min walking distance,the treatment group was better than the control group（P0.05）.Adverse drug reactions was not obvious treatment group.Conclusion Additional drug treatment with trimetazidine on the basis of conventional therapy has notablely therapeutic efficacy on ischemic heart failure in aged people and can improve heart function without severe adverse effect.
