药品零差率销售对安徽省基层医疗机构经济运行的影响分析

摘　　要：目的了解新医改试点前后基层医疗机构总体运营状况和财政补偿现状,为医疗服务价格的进一步改革提供理论依据。方法分层随即抽样选取安徽省首批零差率销售32试点县区中的五县四区,调查基层医疗机构2009年和2010年机构收支构成和医疗服务开展情况,收集的数据进行对比分析。结果新医改试点前后财政补偿大幅增长,但是基层医疗机构收支结余减少;次均医药费用明显下降,但检查费用增加、医疗机构病床使用率下降。结论适当改善地区间财政补偿的比例,调整基层医疗机构规模和技术力量,以达到"大病去医院,小病在社区"就医模式转变。Objective To understand the situation of basic medical institutions operating conditions and financial compensation before and after the pilot new medical health system reform and to provide theoretical basis for medical service price reform.Methods We chose five counties and four areas from 32 pilot counties with stratified sampling in Anhui province,collected and analyzed their institutions revenues/expenditures constitute and medical service in a balance of payments between 2009 and 2010.Results Financial compensation increased sharply,but the balance of medical institutions reduced.Minor medical expenses dropped significantly,but check fee increased and medical institutions bed charges declined.Conclusion It is necessary to improve regional financial compensation for the proportion of basic-level medical institutions,adjust basic-level medical institution scale and technology power,in order to achieve the pattern of ＂serious illness treated in hospital,ailment in community＂.
